个性化护理对降低老年消化道肿瘤病人的PICC堵管和其他并发症的效果探究

Effect of personalized nursing on reducing PICC tube and other complications in elderly patients with digestive tract cancer

目的探究个性化护理对降低老年消化道肿瘤病人的经外周静脉植入中心静脉置管(PICC)堵管和其他并发症的效果。方法选取老年消化道肿瘤并行 PICC 置管患者 80 例,使用随机数字表法将其分为对照组和及观察组,各40例,对照组采用常规护理模式进行护理,观察组进行个性化护理,比较2组护理前后心理状态、自我管理能力及导管留置时间;比较2组护理前后凝血功能、上肢静脉血栓发生率及堵管和其他并发症发生率。结果护理后,观察组心理状态评分、细菌定值率、D-D水平及上肢静脉血栓发生率、堵管和其他并发症发生率显著低于对照组(P<0.05),观察组平均静脉血流流速、置管留置时间及自我管理能力各维度评分显著高于对照组(P<0.05)。结论个性化护理能有效改善老年肿瘤患者心理状态,提高患者自我管理能力,改善其凝血功能,进而降低其上肢静脉血栓、堵管和其他并发症发生率。

Objective To investigate the effect of personalized nursing on the reduction of catheterization and other complications of peripheral venous catheterization(PICC)in elderly patients with gastrointestinal cancer.Methods Eighty elderly patients with digestive tract tumors and PICC catheterization were enrolled,and randomly divided into control group(n=40)and experimental group(n=40).For example,the control group was treated with routine nursing mode.The experimental group was treated with personalized care.The psychological status of the two groups before and after nursing was compared.The self-management ability and catheter indwelling time of the two groups were compared.The coagulation function and upper limb vein of the two groups were compared before and after treatment.Thrombosis rate and incidence of catheterization and other complications.Results After nursing,the psychological state score,bacterial setting rate,D-D level and upper extremity venous thrombosis rate of the experimental group,and the incidence of catheterization and other complications was significantly lower than that of the control group(P<0.05).The average venous blood flow velocity,catheter indwelling time and self-management ability scores in the experimental group were significantly higher than those in the control group(P<0.05).Conclusion Personalized nursing can effectively improve the psychological state of elderly patients with cancer,improve their self-management ability,improve their coagulation function,and reduce the incidence of upper extremity venous thrombosis,occlusion and other complications,which has clinical significance.
