轻型脑外伤患者血浆和肽素、MMP-9水平与头颅CT扫描结果的关系

The correlation between plasma copeptin,MMP-9 level and intracranial lesions on computed tomography in patients with mild traumatic brain injury

目的测定轻型脑外伤(MTBI)患者血浆和肽素、基质金属蛋白酶-9(MMP-9)的含量,并探讨血浆和肽素、MMP-9水平与MTBI患者头颅CT扫描结果的关系。方法根据CT扫描结果将92例MTBI患者分为有颅内损伤的CT扫描阳性组和无颅内损伤的CT扫描阴性组。脑外伤后4h内采取血样,各组血浆和肽素、MMP-9均采用酶联免疫吸附试验检测,应用ROC曲线分析血浆和肽素、MMP-9区分MTBI患者CT扫描阳性与CT扫描阴性的能力。结果MTBI患者92例,79例CT扫描阴性,13例CT扫描阳性。CT扫描阳性组血浆和肽素、MMP-9含量显著高于CT扫描阴性组(P<0.05);血浆和肽素、MMP-9水平有较好区分MTBI患者CT扫描阳性与CT扫描阴性的能力,其ROC曲线下面积(AUC)分别为0.82[95%CI(0.70,0.94)]、0.77[95%CI(0.65,0.89)]。结论测定血浆和肽素、MMP-9水平可以作为MTBI新的辅助检查指标。

Objective To examine the level of plasma copetin and metalloproteinase-9(MMP-9)and detect intracranial lesions on computed tomography(CT)in mild traumatic brain injury(MTBI)patients.Methods 92 patients with MTBI were divided into CT positive group and CT negative group.Blood samples were obtained within 4 h after injury.Plasma copetin and MMP-9 were measured dynamically by ELISA in plasma of CT positive and CT negative patients.Diagnostic value of plasma copetin and MMP-9 was evaluated by receiver operating characterstis(ROC)curve.Results Among the 92 patients with MTBI,13 cases had intracranial lesions,79 cases had no intracranial lesions.Levels of plasma copetin and MMP-9 were signifificantly higher in patients with CT positive(P<0.05).The area under the receiver operating characteristics curve(AUC)for predicting intracranial lesions on CT for copetin was 0.82[95%CI(0.70,0.94)]and for MMP-9 was 0.77[95%CI(0.65,0.89)].Conclusion The plasma levels of copetin and MMP-9 could be used as new indicators of patients with MTBI.
