手指钝性离断伤断指再植的手术治疗效果

Surgical treatment effect of replantation surgery of severed fingers on blunt amputation of fingers

目的分析手指钝性离断伤断指再植的手术治疗效果。方法选择本院2017年4月至2019年4月收治的70例手指钝性离断伤患者为研究对象,均进行显微手术治疗,观察患者的治疗效果、并发症发生情况、满意度、术前及术后3个月生活质量;比较成功组与失败组的手指毁损数目、手指污染面积、受伤时间及可供吻合动静脉数目。结果 70例患者中,24例的治疗效果为优、39例为良、7例为差,优良率为90.00%(63/70)。2例发生感染,并发症总发生率为2.86%(2/70)。20例患者很满意、45例满意、5例不满意,满意度为92.86%(65/70)。术后3个月,患者的环境、社会、心理、及生理评分均显著高于术前(P<0.05)。63例患者再植手术成功,7例患者再植手术失败,成功组的手指毁损数目、手指污染面积、受伤时间及可供吻合动静脉数目均明显优于失败组(P<0.05)。结论断指再植手术治疗手指钝性离断伤的效果显著,并发症较少,患者预后良好,满意度较高,值得借鉴。

Objective To analyze the surgical treatment effect of replantation surgery of severed fingers on blunt severed fingers.Methods From April 2017 to April 2019,70 patients with blunt amputation of fingers admitted in our hospital were selected as the study objects,all of whom were treated with microsurgery.The treatment effect,complications,satisfaction,quality of life before and 3 months after operation were observed;and the number of damaged fingers,contaminated area of fingers,time of injury and number of arteries and veins available for anastomosis were compared between the successful group and the failed group.Results Among the 70 patients,24 cases had excellent treatment effects,39 cases had good results,and 7 cases had poor results,the excellent and good rate was 90.00%(63/70).Infection occurred in 2 cases,,and the total incidence of complications was 2.86%(2/70).A total of 20 patients were very satisfied,45 patients were satisfied,5 patients were not satisfied,and the satisfaction rate was 92.86%(65/70).Three months after surgery,the environmental,social,psychological and physiological scores of the patients were significantly higher than those before surgery(P<0.05).A total of 63 patients had successful replantation surgery and 7 patients had failed replantation surgery.The number of damaged fingers,contaminated area of fingers,time of injury and number of arteries and veins available for anastomosis in the successful group were significantly better than those in the failed group(P<0.05).Conclusion Replantation surgery of severed fingers has a significant effect on patients with blunt amputation of fingers,with fewer complications,good prognosis and higher satisfaction,which is worth learning.
