多b值磁共振弥散加权成像诊断前列腺癌的价值分析

Value of multi-b-value diffusion-weighted MRI in the diagnosis of prostate cancer

目的研究多b值磁共振弥散加权成像(DWI)诊断前列腺癌的价值。方法抽取2015年3月至2019年3月在中国人民解放军联勤保障部队第989医院诊治的60例前列腺病患者为研究对象,所有患者进行多b值(400、600、1000 s/mm^2)DWI检查,观察其信号强度、表观弥散系数(ADC)、负性增强积分(NEI)、增强时间(MTE)值,与多b值DWI对前列腺癌的诊断价值。并对疑似肿瘤患者进行穿刺或手术病理检查。结果60例研究对象中,28例为前列腺癌患者,32例为前列腺增生患者。前列腺增生组b值1000、600、400 s/mm^2信号强度低于前列腺癌组患者,差异有统计学意义(P<0.05);前列腺增生组b值1000、600、400 s/mm^2 ADC值高于前列腺癌组,差异有统计学意义(P<0.05);两组患者MET值比较差异未见统计学意义(P>0.05),前列腺增生组NEI值高于前列腺癌组(P<0.05);当ADC值为0.82×10^-3 s/mm^2,b值为1000 s/mm^2时,鉴别前列腺癌的敏感度为95.32%,特异度为93.54%。结论多b值DWI在前列腺癌诊断中具有较高价值,值得临床推广。

Objective To investigate the value of multi-b-value diffusion-weighted nuclear magnetic resonance imaging(DWI)in the diagnosis of prostate cancer.Methods Sixty patients with prostate cancer in the 989th Hospital of Joint Service Support Force of Chinese People’s Liberation Army from March 2015 to March 2019 were enrolled,and all the patients received the multi-b-value(400,600,1000 s/mm^2)DWI and pathology examinations.The signal strength,apparent diffusion coefficient(ADC),negative enhancement integral(NEI),and mean time to enhance(MTE)were recorded.The value of multi-b-value DWI in the diagnosis of prostate cancer was analyzed.Results Sixty selected subjects included 28 patients with prostate cancer and 32 patients with prostate hyperplasia.The signal intensity of lesions with b-value of 1000,600,and 400 s/mm^2 in the prostatic hyperplasia group were lower than those in the prostate cancer group(P<0.05).The ADC of lesions with b-value of 1000,600,and 400 s/mm^2 in the prostatic hyperplasia group were higher than those in the prostate cancer group(P<0.05).MET had no difference between groups(P>0.05),while the NEI of prostatic hyperplasia group was higher than that in the prostate cancer group(P<0.05).When the ADC value was 0.82×10^-3 s/mm^2 and the b value was 1000 s/mm^2,the sensitivity and specificity of DWI for identifying prostate cancer were 95.32%and 93.54%,respectively.Conclusions Multi-b-value DWI has high value in the diagnosis of prostate cancer and is worthy of clinical promotion.
