血栓弹力图在心脏瓣膜置换术患者抗凝治疗中的应用价值分析

Application value of thromboelastography in anticoagulation therapy for patients undergoing cardiac valve replacement

目的:分析血栓弹力图在心脏瓣膜置换术患者抗凝治疗中的应用价值。方法:选取76例心脏双瓣膜置换术患者为研究对象,将其随机分成观察组和对照组,每组38例,术后两组均采取抗凝治疗,观察组同时应用血栓弹力图的R时间、K时间对用药进行调整,对照组则不应用,比较两组凝血功能。结果:两组术后3 d、7 d凝血酶原时间、活化部分凝血酶时间、纤维蛋白原和D二聚体均高于术前,观察组术后上述凝血功能指标均低于对照组,组间比较,差异有统计学意义(P<0.05)。结论:血栓弹力图动态监测为心脏双瓣膜置换术患者抗凝治疗提供指导。

Objective To analyze the value of thromboelastography in anticoagulation therapy for patients undergoing cardiac valve replacement.Method 76 patients with heart double valve replacemen in our hospital were studied.And they were randomly divided into observation group and control group.38 cases in each group.After operation,anticoagulation therapy was adopted in both groups,The R time and K time of thromboelastogram were used to adjust the medication in the observation group,but not in the control group.The coagulation function of the two groups were compared.Results The prothrombin time,activated partial thrombin time,fibrinogen and D dimer of the two groups at 3 and 7 days after operation were higher than those before operation.The above coagulation function indexes of the observation group were lower than those of the control group after operation,and there was a significant difference between the two groups(P<0.05).Conclusion Dynamic monitoring of thromboelastography provides guidance for anticoagulation therapy in patients undergoing heart double valve replacemen.
