参苓白术散加减联合心理护理治疗轻中度克罗恩病临床研究

Clinical Study on Modified Shenling Baizhu Powder Combined with Psychological Nursing for Mild and Moderate Crohn’s Disease

目的:观察参苓白术散加减联合心理护理治疗轻中度克罗恩病的临床疗效。方法:将轻中度克罗恩病患者60例随机分为对照组和观察组,每组30例;对照组给予泼尼松龙、硫唑嘌呤片联合常规护理治疗,观察组在对照组治疗的基础上加用参苓白术散加减及心理护理治疗;2组均连续治疗8周,观察比较2组简化克罗恩病活动指数(CDAI)评分、Best CDAI评分、临床疗效以及红细胞沉降率(ESR)、C-反应蛋白(CRP)水平情况。结果:临床缓解率观察组为76.67%,对照组为46.67%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组简化CDAI评分、Best CDAI评分均较治疗前明显降低(P<0.05);且观察组简化CDAI评分、Best CDAI评分均低于对照组(P<0.05)。治疗后,2组ESR、CRP水平均较治疗前明显降低(P<0.05);且观察ESR、CRP水平均低于对照组(P<0.05)。结论:在西医常规治疗的基础上,参苓白术散加减治疗联合心理护理干预轻中度克罗恩病疗效确切,效果优于单纯西药治疗。

Objective:To observe the clinical effect of Shenling Baizhu powder combined with psychological nursing for mild and moderate Crohn’s disease. Methods:A total of 60 cases of patients with mild and moderate Crohn’s disease were randomly divided into the control group and the observation group, 30 cases in each group. The control group was given prednisolone and azathioprine tablets combined with routine nursing treatment,and the observation group was additionally given modified Shenling Baizhu powder and psychological nursing treatment based on the treatment of the control group. Both groups were treated for eight weeks. The scores of simplified Crohn’s Disease Activity Index(CDAI) and Best CDAI, the clinical effect as well as the levels of erythrocyte sedimentation rate(ESR) and C-reactive protein(CRP) in the two groups were observed and compared. Results:The clinical remission rate was 76.67% in the observation group and 46.67% in the control group, the difference being significant(P<0.05). After treatment, the scores of simplified CDAI and Best CDAI in the two groups were significantly decreased when compared with those before treatment(P<0.05);and the above scores in the observation group were lower than those in the control group(P<0.05). After treatment,the levels of ESR and CRP in the two groups were significantly decreased when compared with those before treatment(P<0.05);and the above levels were lower than those in the control group(P<0.05). Conclusion:Based on routine western medicine treatment,the therapy of modified Shenling Baizhu powder combined with psychological nursing has definite curative effect in treating mild and moderate Crohn’s disease,whose effect is better than that of simple western medicine..
