摘要
目的探讨乳腺导管内癌(DCIS)患者超声造影定量参数与肿瘤组织中侵袭基因表达的关联性及于疗效评估中的价值。方法选取2017年2月至2019年2月我院80例DCIS患者作为研究组,另选同期80例乳腺良性肿瘤患者作为对照组,均采取超声造影检查,并检测肿瘤组织中侵袭基因[叉头蛋白F1(FOXF1)、N-myc下游调节基因1(NDRG1)、GRB2相关结合蛋白(GAB2)]表达量。研究组采取4个周期新辅助放化疗。对比两组超声造影定量参数[曲线下面积(AUC)、达峰强度(PI)]、肿瘤组织中侵袭基因(FOXF1、NDRG1、GAB2)表达量、研究组不同疗效患者超声造影定量参数水平,应用Spearman相关分析探究DCIS患者超声造影定量参数与肿瘤组织中侵袭基因表达及临床疗效之间关系。结果(1)超声造影定量参数:研究组AUC、PI值高于对照组,差异有统计学意义(P<0.05)。(2)肿瘤组织中侵袭基因表达量:研究组肿瘤组织中FOXF1、GAB2 mRNA表达量高于对照组,NDRG1 mRNA表达量低于对照组,差异有统计学意义(P<0.05)。(3)不同疗效超声造影定量参数水平:研究组治疗有效患者AUC、PI值低于治疗无效患者,差异有统计学意义(P<0.05)。(4)相关性分析:DCIS患者超声造影定量参数AUC、PI值与肿瘤组织中FOXF1、GAB2表达呈正相关,与肿瘤组织中NDRG1表达、疗效呈负相关,差异有统计学意义(P<0.05)。结论DCIS患者超声造影定量参数与肿瘤组织中侵袭基因表达及疗效具有明显相关性,可作为评估DCIS患者肿瘤侵袭程度及治疗效果的重要指标,为临床合理制定治疗方案提供科学参考,具有较高应用价值。
Objective To investigate the relationship between the quantitative parameters of contrast-enhanced ultrasound and the expression of invasive genes in tumor tissues in patients with intraductal carcinoma of the breast(DCIS)and its value in the evaluation of curative effect.Methods From February 2017 to February 2019,80 patients with DCIS in our hospital were selected as the study group.Eighty patients with benign breast tumors were selected as the control group.Ultrasonography was performed and the invasive gene in the tumor tissue was detected.The expression levels of protein F1(FOXF1),N-myc downstream regulatory gene 1(NDRG1),and GRB2-related binding protein(GAB2)].The study group took4 cycles of neoadjuvant chemoradiation.The contrast parameters of ultrasound contrast(area under the curve(AUC),peak intensity(PI)),the expression of invasive genes(FOXF1,NDRG1,GAB2)in tumor tissues,and the quantitative parameters of contrast-enhanced ultrasound in patients with different curative effects were applied.Spearman correlation analysis explored the relationship between the quantitative parameters of contrast-enhanced ultrasound in DCIS patients and the expression of invasive genes and clinical efficacy in tumor tissues.Results(1)Quantitative parameters of contrast-enhanced ultrasound:The AUC and PI values of the study group were higher than those of the control group,and the difference was statistically significant(P<0.05).(2)Invasion gene expression in tumor tissues:The expression levels of FOXF1 and GAB2 mRNA in the tumor tissues of the study group were higher than those in the control group,and the expression of NDRG1 mRNA was lower than that in the control group(P<0.05).(3)Quantitative parameters of different curative ultrasound contrast:The AUC and PI values of the patients in the study group were lower than those of the patients with treatment failure,the difference was statistically significant(P<0.05).(4)Correlation analysis:The quantitative parameters of AUC and PI in patients with DCIS were positively correlated with the expression of FOXF1 and GAB2 in tumor tissues,and negatively correlated with the expression and efficacy of NDRG1 in tumor tissues.The difference was statistically significant(P<0.05).Conclusion The quantitative parameters of contrast-enhanced ultrasound in DCIS patients have obvious correlation with the expression and efficacy of invasive gene in tumor tissues.It can be used as an important index to evaluate the degree of tumor invasion and therapeutic effect in patients with DCIS.It provides a scientific reference for clinically rational treatment planning and has high applicable value.
