835例百日咳患儿PTIgG水平的影响因素分析 被引量:8

摘要 目的探讨影响百日咳患儿抗百日咳杆菌毒素抗体IgG(PTIgG)水平的因素。方法收集2012年1月至2016年12月期间西安市儿童医院收治的835例病原学确诊的百日咳患儿的病例资料,按性别、年龄、免疫状态、咳嗽病程分组,比较各组PTIgG检测水平,采用多因素Logistic回归分析影响PTIgG的因素。结果 835例患儿中男孩475例(56.89%),女孩360例(43.11%)。年龄<1岁共655例(78.44%),总体咳嗽病程(16.21±8.70)天。患儿中未接种疫苗116例(13.89%),接种1针次21例(2.51%),接种2针次42例(5.03%),接种3针次83例(9.94%),接种4针次65例(7.78%)。患儿PTIgG测量结果为56.00(2.69~125.00)IU/mL,PTIgG阳性组327例,阴性组508例,百日咳患儿中年龄大、免疫接种剂次多、病程长者PTIgG更易得阳性结果,其OR值及95%CI分别为1.851(1.474~2.325)、1.837(1.140~2.961)、1.076(1.054~1.099),均P<0.05。结论病程、年龄、疫苗接种水平是百日咳患儿PTIgG水平的独立影响因素。 Objective To explore the influencing factors of anti-pertussis toxin immunoglobulin G(PTIgG)level in children with pertussis.Methods Data of 835children etiologically confirmed with pertussis and admitted to Xi'an Children's Hospital from January 2012to December 2016were collected.All of them were grouped according to gender,age,immune status and cough course.PTIgG detection level of each group was compared.Results Among the 835children,475 were boys(56.89%)and 360 were girls(43.11%).A total of 655patients(78.44%)were less than 1years old.The duration of total cough was(16.21±8.70)days.Among the children,116(13.89%)were not vaccinated,21(2.51%)were vaccinated with one dose,while 42(5.03%)with two doses,83(9.94%)with three doses,and 65(7.78%)with four doses.The PTIgG level in children were 56.00(2.69~125.00)IU/mL,327patients in the PTIgG positive group and 508patients in the negative group.The older,the more times the immunization agents and the longer the duration of pertussis,the more likely PTIgG to get the positive results,with OR values and 95%CI were 1.851(1.474~2.325),1.837(1.140~2.961)and 1.076(1.054~1.099),respectively,all P<0.05.Conclusion Cough duration,age and vaccination level are independent influencing factors of PTIgG level in children with pertussis.
