超声引导下改良胸膜活检钳在兔胸膜活检中的效果

Application of ultrasound-guided modified pleural biopsy forceps in rabbit pleural biopsy

目的探讨使用改良胸膜活检钳在兔胸膜活检中的有效性和安全性。方法(1)选取40只新西兰兔分实验组和对照组,超声引导下行右侧胸腔积液造模,实验组使用改良胸膜活检钳、对照组使用钝头钩针(Cope针),分别在少量胸腔积液及多量胸腔积液时行兔胸膜活检,对比观察活检效果。(2)选取20只新西兰兔分实验组和对照组,超声引导下行右侧胸腔增厚胸膜造模。实验组使用改良胸膜活检钳行增厚胸膜活检,对照组使用Cope针行增厚胸膜活检,对比观察活检效果。结果使用改良胸膜活检钳行兔胸膜活检,无论是正常胸膜还是增厚胸膜,其获取胸膜组织成功率均明显高于使用Cope针;在少量胸腔积液情况下,使用改良胸膜活检钳行兔胸膜活检,亦能取得较为满意组织,同时多量气胸发生率较后者低。结论超声引导下使用改良胸膜活检钳行胸膜活检相对于传统Cope针胸膜活检是一种更加安全有效的方法。

Objective To evaluate the diagnostic value and safety of application of modified pleural biopsy forceps in rabbit pleural biopsy.Methods Forty New Zealand rabbits were selected to establish right pleural effusion model under ultrasound guidance,and divided into experimental group and control group.The experimental group receivedpleural biopsy with modified pleural biopsy forceps and the control group receivedpleural biopsy with cope needle.Pleura thickening model was established on the basis of pleural effusion model in 20 rabbits.Thickened pleural biopsy was performed with modified pleural biopsy forceps in the experimental group and with Cope needle was applied in the control group.Results Under normal or thickened pleura,the successful rate of obtaining pleural tissue by using the modified pleural biopsy forceps was significantly higher than that by using the Cope needle.In the case of a small amount of pleural effusion,the satisfactory tissue could also be obtained by using the modified pleural biopsy forceps,and the incidence of multiple pneumothorax was lower than that of the latter.Conclusion It is a safe and effective method to use the modified biopsy forceps under the guidance of ultrasound.
