内热针联合常规药物治疗阳虚寒痹型膝骨性关节炎的临床疗效观察

目的观察内热针联合常规药物治疗阳虚寒痹型膝骨性关节炎(KOA)的临床疗效。方法采用随机分组法选择2017年12月至2019年4月于本院收治的40例KOA患者为观察对象,分为对照组和治疗组,两组患者在住院期间的基础治疗方法相同,给予抗炎镇痛及润滑关节对症处理,两组分别采用普通针灸针及内热式针灸针治疗1次/d,1周治疗5次,共计3周。比较两组患者治疗前后膝关节疼痛评分、日常生活能力评分及血清中肿瘤坏死因子-α(TNF-α)、白细胞介素-1(IL-1)指标变化及临床疗效。结果两组患者在总疗程后关节疼痛、活动能力及TNF-α、IL-1含量均较前治疗前明显改善(P<0.05),且治疗组效果均优于对照组。在晨僵项目的比较中,对照组在治疗后未见明显好转,而治疗组有明显改善。总疗程后,对照组的总有效率为50%,而治疗组的总有效率为80%,治疗组的有效率优于对照组。结论内热针治疗能改善阳虚寒痹型KOA患者的症状和体征。

Objective To observe the clinical efficacy of internal heat injection combined with conventional drugs in the treatment of cerebral collateral arthritis(KOA).Methods A randomized grouping method was used to select 40 patients with KOA who were admitted to our hospital from December 2017 to April 2019.They were divided into control group and treatment group.The basic treatment methods of the two groups of patients during hospitalization were the same,and the anti-inflammatory analgesic and lubricated joint symptomatic treatment were given.The two groups of patients were treated with ordinary acupuncture needles and internal heat acupuncture needles once a day for 5 times a week for a total of three weeks.The knee pain score,daily living ability score,serum TNF-α,IL-1 index changes and clinical efficacy were compared between the two groups.Results The joint pain,walking ability and TNF-α,IL-1 levels in the two groups were better than those before the treatment(P<0.05),and the treatment group was better than the control group.In the comparison of the morning stiffness project,the control group did not significantly improve after treatment,while the treatment group showed significant improvement.The total effective rate of the control group was 50%,and the total effective rate of the treatment group was 80%.The effective rate of the treatment group was better than that of the control group.Conclusion Internal heat acupuncture treatment can improve the symptoms and signs of patients with yang deficiency and cold sputum type KOA.
