腹腔镜肾癌根治术在肾癌患者中的疗效观察

Effect of Laparoscopic Radical Nephrectomy in Patients With Renal Carcinoma

目的探讨腹腔镜肾癌根治术治疗肾癌的效果。方法研究对象为本院2018年1月—2020年1月收治的44例肾癌患者,根据不同手术方式将其分为开放组(n=22,行开放性肾癌根治术治疗)和腹腔镜组(n=22,行腹腔镜肾癌根治术治疗)。对比两组围手术期指标、手术前后应激反应[C反应蛋白(C-reactive protein,CRP)、皮质醇(cortisol,Cor)、去甲肾上腺素(norepin ephrine,NE)、丙二醛(malondialdehyde,MDA)]改善情况。结果腹腔镜组的手术时间、胃肠功能恢复时间、下床活动时间及住院时间均短于开放组,术中出血量少于开放组(P<0.05);且与开放组比较,腹腔镜组术后的CRP、Cor、MDA、NE水平均明显提高(P<0.05)。结论腹腔镜肾癌根治术能有效缓解肾癌患者术后机体疼痛程度,应激反应较小,有利于促进患者尽早康复,缩短住院时间。

Objective To investigate the effect of laparoscopic radical nephrectomy on renal carcinoma.Methods 44 patients with renal cancer admitted to our hospital from January 2018 to January 2020 were divided into open group(n=22,open radical nephrectomy)and laparoscopic group(n=22,laparoscopic radical nephrectomy)according to different surgical methods.The improvement of perioperative indicators,C-reactive protein(CRP),cortisol(Cor),norepin ephrine(NE)and malondialdehyde(MDA)were compared between the two groups.Results The operation time,gastrointestinal function recovery time,time of getting out of bed and hospital stay of the laparoscopic group were shorter than those of the open group,and the intraoperative blood loss was less than that of the open group(P<0.05).Moreover,compared with the open group,the postoperative CRP,Cor,MDA and NE levels in the laparoscopic group were significantly increased(P<0.05).Conclusion Laparoscopic radical nephrectomy can effectively relieve the postoperative pain of renal cancer patients,reduce the stress response,promote the recovery of patients as soon as possible,and shorten the length of hospital stay.
