磁共振与超声对单纯性及化脓性阑尾炎的诊断比较

MRI and Ultrasound Diagnosis of Simple Appendicitis and Suppurative Appendicitis

目的对比磁共振检查与超声检查在单纯性阑尾炎及化脓性阑尾炎的诊断及鉴别诊断中的应用情况。方法选择2018年9月—2019年6月的107例阑尾炎切除术患者为研究对象。根据术后病理分为单纯性阑尾炎组51例及化脓性阑尾炎组56例,所有患者术前均接受磁共振及腹部超声检查,对比两种检查方法的诊断阳性率。结果单纯性阑尾炎组患者C反应蛋白(C-reactive protein,CRP)、白细胞计数、中性粒细胞百分比低于化脓性阑尾炎组(P<0.05)。51例单纯性阑尾炎患者磁共振诊断正确率为94.12%、超声诊断正确率72.55%;56例化脓性阑尾炎磁共振诊断正确率为92.86%、超声诊断正确率83.93%。比较差异具有统计学意义(P<0.05)。结论磁共振在单纯性阑尾炎及化脓性阑尾炎的诊断及鉴别诊断中具有较高的准确率。

Objective To compare the application of magnetic resonance examination and ultrasound examination in the diagnosis and differential diagnosis of simple appendicitis and suppurative appendicitis.Methods 107 cases of appendicitis resection from September 2018 to June 2019 were selected as the study object.According to the postoperative pathology,51 cases were divided into simple appendicitis group and 56 cases were purulent appendicitis group.All patients underwent magnetic resonance and abdominal ultrasound examination before operation,and the diagnostic positive rates of the two examination methods were compared.Results The CRP,WBC,and neutrophil percentage in patients with simple appendicitis were significantly lower(P<0.05).51 patients with simple appendicitis had a correct diagnosis rate of 94.12%for magnetic resonance imaging and 72.55%for ultrasound diagnosis;56 cases of suppurative appendicitis had a diagnostic accuracy of 92.86%and an ultrasound diagnosis accuracy of 83.93%.The comparisons were statistically significant(P<0.05).Conclusion There is a high accuracy in the diagnosis and differential diagnosis of simple appendicitis and suppurative appendicitis by MRI.
