踝关节应力试验在踝关节旋后-外旋型骨折患者中的应用效果

Application effect of ankle joint stress test in patients with ankle joint supination-external rotation fracture

目的探讨踝关节应力试验在踝关节旋后-外旋型骨折患者中的临床效果。方法选取2018年10月~2019年5月我院收治的56例踝关节旋后-外旋型骨折患者为研究对象,根据随机数字表法将其分为对照组、观察组,每组各28例。对照组患者采用X线分型,观察组患者采用X线+踝关节应力试验分型。比较两组患者的美国矫形外科足踝协会(AOFAS)踝-后足评分、治疗效果、生活质量。结果观察组患者的踝关节应力试验(+)18例(64.29%),应力试验(-)10例(35.71%);治疗后3个月,观察组患者的AOFAS踝-后足评分高于对照组,差异有统计学意义(P<0.05);观察组患者的治疗总有效率为96.43%,高于对照组的78.57%,差异有统计学意义(P<0.05);观察组患者的精神健康、情感职能、社会功能、活力、总体健康、躯体疼痛、生理职能、生理功能等生活质量评分均高于对照组,差异有统计学意义(P<0.05)。结论在关节旋后-外旋型骨折患者治疗中开展踝关节应力试验,能够明确腓骨骨折合并三角韧带损伤分型,并通过针对性治疗,改善踝、足功能,提升治疗效果,对患者的生活质量具积极意义。

Objective To investigate the clinical effect of ankle stress test in patients with ankle joint supination-external rotation fracture.Methods A total of 56 patients with ankle joint supination-external rotation fracture admitted to our hospital from October 2018 to May 2019 were selected as subjects.According to the random number table method,the patients were divided into control group and observation group,28 cases in each group.The control group was classified by X-ray,while the observation group was classified by X-ray+ankle stress test.The American orthopedic foot and ankle association(AOFAS)ankle-posterior foot score,treatment effect,and quality of life were compared between the two groups.Results In the observation group,there were 18 cases(64.29%)of ankle stress test(+)and 10 cases(35.71%)of ankle stress test(-).Three months after treatment,the AOFAS ankle-posterior foot score in the observation group was higher than that in the control group,with statistically significant difference(P<0.05).The total effective rate of the observation group was 96.43%,higher than that of the control group(78.57%),and the difference was statistically significant(P<0.05).The scores of mental health,emotional function,social function,vitality,overall health,physical pain,physiological function and physiological function of the patients in the observation group were all higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion The ankle joint stress test of ankle joint in the treatment of patients with ankle joint supination-external rotation fracture can clarify the classification of fibula fractures complicated with triangular ligament injuries,improve ankle and foot functions through targeted treatment,improve the treatment effect,and have positive significance for the quality of life of patients.
