盆腔孤立性纤维瘤的临床病理特点及MRI分析

Clinicopathological features and MR analysis of pelvic solitary fibroma

目的探讨盆腔孤立性纤维瘤(solitary fibrous tumors,SFT)的临床病理特点及MRI表现。方法收集经手术病理证实的9例盆腔SFT的临床资料,结合临床分析MRI表现。结果9例患者均为单发病灶,呈圆形、类圆形或椭圆形软组织肿块,边界清晰,与盆壁肌肉信号相比,T1WI以等信号为主,T2WI多呈混杂信号,脂肪抑制序列呈明显高信号,增强呈明显均匀(n=4)或不均匀强化(n=5)。结论盆腔SFT临床多无特殊症状,MRI主要表现为单发肿块,边界清楚,由于成份多样、合并囊变及粘液样变致T2WI信号混杂,增强病灶明显强化或不均匀多种方式强化。

Objective To investigate the clinicopathological features and MR findings of pelvic solitary fibroma,so as to improve the imaging diagnossis.Methods The clinical,pathological and MR data of 9 cases with pathologically confirmed SFT of pelivc were collected and the MR findings were analyzed retrospectively.Results All 9 patients had a single lesion,with a round or oval soft tissue masses,sharpness of border.Compared with pelvic wall muscle signals,all lesions displayed equal signal on T 1WI,mixed signal on T 2WI,obvious hyperintensity on STIR.The enhancement was obviously uniform(n=4)or uneven(n=5).Conclusion Pelvic SFT has no special symptoms in clinic,the main features of MR are single mass with clear boundary.Due to the diversity of components,combined cystic changes and mucus-like changes,the T 2WI signal is mixed,and the enhanced lesions are obviously enhanced or unevenly enhanced in various ways,the possibility of pelvic SFT should be taken into account.
