摘要
目的研究创伤性颅脑损伤急性期垂体前叶激素及甲状腺激素水平变化。方法选择2018-06—2019-08间收治的90例颅脑损伤患者,按照GCS评分分为轻型组36例、中型组29例、重型组25例。记录3组患者伤后第1天的垂体前叶激素(GH、ATCH、PRL、FSH、LH、TSH)和甲状腺激素(TT3、FT3、TT4、FT4)水平,探讨严重程度与两种激素变化的相关性。结果中型、重型组GH、ATCH、PRL、FSH较轻型组增高,TT3、FT3较轻型组降低,差异均有统计学意义(P<0.05)。重型组GH、ATCH、PRL、FSH较中型组增高,TT3、FT3较中型组降低,差异有统计学意义(P<0.05)。结论神经内分泌激素水平变化对于评估颅脑损伤患者的损伤程度具有重要的价值,检测颅脑损伤患者垂体前叶激素水平和靶腺之一的甲状腺相关激素水平的变化,可评估患者的损伤严重程度。
Objective To study the changes of anterior pituitary hormone and thyroid hormone in the acute stage of traumatic brain injury.Methods From 2018-06 to 2019-08,90 patients with craniocerebral injury were divided into 36 cases in light group,29 cases in medium group and 25 cases in severe group according to GCS score.The levels of anterior pituitary hormone(GH、ATCH、PRL、FSH、LH、TSH)and thyroid hormone(TT3、FT3、TT4、FT4)were recorded on the first day after injury in 3 groups,and the correlation between severity and changes of two hormones was explored.Results The GH、ATCH、PRL、FSH of medium and heavy group was higher than that of light group,and the TT3、FT3 was lower than that of light group,and the difference was statistically significant(P<0.05).The GH、ATCH、PRL、FSH of heavy group was higher than that of medium group,and the TT3、FT3 was lower than that of medium group(P<0.05).Conclusion The change of neuroendocrine hormone level is of great value to assess the degree of injury in patients with craniocerebral injury.The change of pituitary hormone level and thyroid hormone level in one of the target glands in patients with craniocerebral injury can assess the severity of injury in patients.
