摘要 目的:研究血糖波动与进展性脑梗死(PCI)的关系。方法:选取2016年9月至2018年10月肇庆市第一人民医院收治的136例急性脑梗死(ACI)患者作为研究对象,根据病情严重程度将72例急性PCI患者纳入观察组,64例急性稳定性脑梗死患者纳入对照组。计算两组日内多点血糖的标准差(SDBG)、日内最大与最小血糖值之差(LAGE)、餐后血糖波动幅度(PPGE)、平均血糖波动幅度(MAGE),采用Pearson相关性分析PCI患者美国国立卫生研究院卒中量表(NIHSS)评分与血糖波动的关系。结果:观察组NIHSS评分以及SDBG、LAGE、PPGE、MAGE均明显高于对照组,差异均具有统计学意义(P<0.05);Pearson相关性分析显示PCI患者NIHSS评分与SDBG、LAGE、PPGE、MAGE呈显著正相关关系。结论:PCI患者NIHSS评分与血糖波动关系密切,血糖波动幅度越大,患者NIHSS评分越高。 Objective To study relationship between glucose fluctuation and progressive cerebral infarction(PCI).Methods A total of 136 patients with acute cerebral infarction(ACI)who were admitted to the hospital from September 2016 to October 2018 were enrolled.According to disease severity,72 cases with acute PCI were included in observation group,while 64 cases with acute stable cerebral infarction(SCI)were included in control group.The standard deviation of blood glucose(SDBG),largest amplitude of glycemic excursions(LAGE),postprandial glucose excursion(PPGE)and mean amplitude of glycemic excursions(MAGE)in both groups were calculated,Pearson correlation analysis of the relationship between NIHSS score and glucose fluctuation in PCI patients.Results The score of National Institutes of Health Stroke Scale(NIHSS),SDBG,LAGE,PPGE and MAGE in observation group were significantly higher than those in control group(P<0.05).Pearson correlation analysis showed that NIHSS score was significantly positively correlated with SDBG,LAGE,PPGE and MAGE in PCI patients(P<0.05).Conclusion NIHSS score is closely related to glucose fluctuation in PCI patients.The greater the glucose fluctuation amplitude,the higher the NIHSS score.
