精准院前预检分诊在新型冠状病毒肺炎定点收治医院的应用

Application of precise pre-hospital pre-examination and triage in the Designated hospital of COVID-19

目的:探讨精准院前预检分诊在新型冠状病毒肺炎(COVID–19)定点收治医院的应用效果。方法:选择深圳市第三人民医院2020年1月27日至2月27日疫情防控期间的就诊患者,通过体温检测、流行病学调查、优化分诊流程、合理配置人力资源、对患者进行疾病相关防护知识宣传等精准院前预检分诊防控措施,统计就诊患者与发热患者总数、发热患者救治的及时性和效果,并进行数据分析。结果:就诊患者总数8767例,预检分诊441例,其中,体温≥37.3℃者、再度人工水银体温计核查的发热患者220例,占预检分诊49.89%。将有流行病接触史通过快速通道转入发热门诊1区74例,其余转入发热门诊2区146例,两者差异具有统计学意义(P<0.05)。没有发生传染病漏诊、漏报、交叉感染、医务人员感染现象。结论:精准院前预检分诊在COVID–19定点收治医院的应用,能有效筛查发热患者及相关流行病接触史,使患者得到及时救治,同时也保证了综合门诊其他专科患者的就诊安全。

Objective To investigate the application effect of accurate pre-hospital triage in COVID-19 designated hospital.Methods From January 27,2020 to February 27,Select patients on Shenzhen third hospital during the epidemic prevention and control,through the temperature detection,epidemiological investigation,optimizing the triage process,the rational allocation of human resources,to the patients with disease protection knowledge propaganda and so on precise pre-hospital preview triage measures,observation statistics,the total number of patients with fever patients fever patients timely treatment and data analysis.Results A total of 8767 patients were treated,and 441 pre-diagnosed and classified,among them,220 cases of fever patients with body temperature≥37.3℃and rechecked by artificial mercury thermometers accounted for 49.89%of the pre-examination triage.74 cases with epidemiological contact history were transferred to District 1 fever clinic 1 through the fast track,and the remaining 146 cases were transferred to District 2 fever clinic.The difference was statistically significant(P<0.05).There were no missed diagnoses,missed reports,cross-infections,and infections by medical personnel.Conclusion the application of accurate pre-hospital pre-hospital triage in COVID–19 hospitals can effectively screen patients with fever and their contact history of relevant epidemic diseases,so that fever patients can get timely treatment,and meanwhile ensure the safety of patients with other specialties in the comprehensive outpatient department.
