舒缓医学在城乡社区的实践与社会支持作用

Studying on The Practice and Social Support of Palliative Medicine in Urban and Rural Communities

目的探讨舒缓医学在城乡社区的实践对晚期癌症患者的社会支持作用。方法晚期癌症患者舒缓医学干预后,测评生活质量和家属的满意度,比较癌症住院患者的总费用及每日费用,进行统计学处理。结果实施舒缓医学服务初期和1个月后,晚期癌症患者生活质量平均总分分别为39.91±4.09分和42.94±3.71分,t=6.98,P<0.01。舒缓医学干预1个月后家属的满意度为96.00±6.39分。居家缓医学干预的晚期癌症患者与同期肿瘤科住院的晚期癌症患者的住院平均总费用分别为:3103.49±1644.99(元)和36416.89±10362.69(元),t=23.05,P<0.01;每日医疗费用分别为48.28±16.76(元)和1545.37±896.43(元),t=21.48,P<0.01。结论舒缓医学在城乡社区的实践,能节省医疗资源,舒缓患者及其家属的心理压力,有效提升贫困癌症患者的生活质量,有积极的社会支持作用。

Objective To explore the social support of palliative medicine on patients with later stage cancer in urban and rural communities.Methods After palliative medical interventions on patients with later stage cancer,their life quality and their families’satisfaction degree was evaluated,and their total and daily expenses were statistically analyzed.Results Before and after intervention in one month,the average total score of life quality of patients with later stage cancer was 39.91±4.09 and 42.94±3.71,respectively.After the intervention,their families’satisfaction degree was 96.00±6.39.The average total hospitalization expenses of patients receiving palliative medical intervention at home and hospitalized patients in the same period was 3103.49±1644.99 RMB Yuan and 36416.89±10362.69 RMB Yuan,respectively.And their daily medical expense was 48.28±16.76RMB Yuan and 1545.37±896.43RMB Yuan,respectively.Conclusion The practice of palliative medicine on cancer patients in urban and rural communities can save medical resources,relieve the psychological pressure of patients and their families,effectively improve their life quality,which has positive social support role.
