Clinical Significance of ALDH1 and Ki67 Expression in Women with Breast Carcinoma

摘要 目的:乳腺癌在全球女性中是最常见的恶性肿瘤,并且中国女性乳腺癌的发病率正逐年升高。ALDH1是由原始干细胞分化出来的一种肿瘤标记物,能够氧化细胞内的醛,将视黄醇转化为视黄酸,具有肿瘤干细胞的活性。Ki67的表达水平可作为一个评价预后的指标,用作持续监测患者在接受了全身治疗之后的疾病发展情况及复发可能。本研究主要探讨女性乳腺癌组织中ALDH1及Ki67之间的表达特点及其与临床病理特征之间的联系。方法:对ALDH1和Ki67之间的表达特点与乳腺癌临床病理特征之间的关系,以及ALDH1和Ki67分别在乳腺癌组织中及正常组织中的表达特点进行评价,检验方法为卡方检验,以P<0.05具有统计学意义。ALDH1和Ki67之间表达关系的评估用Spearman秩的相关性进行分析。结果:与正常乳腺组织相比,ALDH1和Ki67在乳腺癌组织呈高表达,差异有统计学意义(P<0.05)。ALDH1在乳腺癌分化越差的组织中,表达的阳性率越高。同时,在III/IV期乳腺癌组织中,ALDH1的表达水平明显高于I/II期的乳腺癌患者。在淋巴结转移患者中的淋巴结组织中,ALDH1和Ki67的阳性表达率明显高于没有淋巴结癌转移的患者。结论:ALDH1可以作为乳腺癌的一种特殊的标记物。ALDH1和Ki67在女性乳腺癌组织中的表达呈正相关性,这对于指导患者治疗及提示预后或许具有重要的临床应用意义。 Objective:Breast cancer is the most prominent cancer among women worldwide.ALDH1,catalyzing the oxidation of intracellular aldehydes and converting retinol into retinoic acid,serves as a biomarker of early stem cell differentiation.Ki67 levels are prognostic or residual risk biomarkers after primary therapy and can predict the effects of systemic therapies,or to monitor patients for sustained response or resistance to the administered therapies.This study aimed to investigate the correlation between ALDH1 and Ki67 expression and clinicopathological parameters among women with breast cancer.Methods:Associations between ALDH1 and Ki67 and clinicopathological parameters of breast cancer and that between ALDH1 and Ki67 expression in breast cancer and healthy tissue were evaluated using the chi-squaretest.A P-value less than 0.05 was considered statistically significant.The correlation between ALDH1 and Ki67 expression was assessed via Spearman’s rank correlation analysis.Results:ALDH1 and Ki67 were upregulated in breast cancer tissue rather than in normal breast tissue(P<0.05).Furthermore,ALDH1 expression was further upregulated with an advancement in the breast cancer grade,i.e.,ALDH1 expression levels were higher in patients with stage III/IV breast cancer than in those with stages I/II breast cancer.Furthermore,ALDH1 and Ki67 were upregulated in the presence of lymphatic metastasis rather than in the absence of lymphatic metastasis.Conclusion:ALDH1 may be considered a pathognomonic marker for breast cancer.ALDH1 and Ki67 expression are significantly positively correlated in women with breast cancer.
