1623例良性阵发性位置性眩晕的临床特征及护理记录要点

Clinical Characteristics and Nursing Records of 1623 Cases of Benign Paroxysmal Positional Vertigo

目的观察良性阵发性位置性眩晕的临床特征和疗效分析为制定护理记录要点提供依据,以便采取有效的护理措施。方法回顾性性分析北京某三级甲等医院2018年1月1日-2019年12月31日符合诊断标准的1623例患者资料。统计分析患者基本信息,病案首页ADL量表得分及病史,按照良性阵发性位置性眩晕诊断标准分型统计患者半规管受累情况。结果 1623例患者平均年龄为(60.6±11.4)岁,男女比1:1.65,具有合并症患者为1151例,占总例数的70.92%。临床主要分类为后半规管管结石症945例占58%,水平半规管管石症392例,占24%,水平半规管嵴帽结石症258例,占16%,混合型良性阵发性位置性眩晕占1%。结论良性阵发性位置性眩晕的临床特征是女性患者多于男性患者,中老年高发,后半规管管石型为主,并有64%患者有合并症,在护理记录上应在注意记录选择合适的量表评估病情,注意疾病相关记录以及加强心理护理和健康宣教。

Objective To observe the clinical features and curative effect of benign paroxysmal positional vertigo and analyze the main points of nursing records.Methods The data of 1623 patients who met the diagnostic criteria in a Grade 3 A hospital in Beijing from January 1,2018 to December 31,2019 were retrospectively analyzed.The basic information of the patients,the score of the ADL scale on the first page of the medical record and the medical history were statistically analyzed,and the semicircular canal involvement was counted according to the diagnostic criteria of benign paroxysmal positional vertigo.Results The average age of 1623 patients was(60.6±11.4)years,the ratio of male to female was 1.65,and there were 1151 patients with complications,accounting for 70.92%of the total.The main clinical categories are posterior semicircular canal lithiasis 945 cases(58%),horizontal semicircular canal calculus 392 cases(24%),horizontal semicircular canal crest cap lithiasis 258 cases(16%).Mixed benign paroxysmal positional vertigo accounted for 1%.Conclusion The clinical characteristics of benign paroxysmal positional vertigo are that female patients are more than male patients,middle-aged and elderly patients have a high incidence,posterior semicircular tube stone type is the main type,and 64%of the patients have complications.In the nursing records,we should select the appropriate scale to evaluate the condition,pay attention to the disease-related records and strengthen psychological nursing and health education.
