摘要
目的观察乌司他丁与低分子肝素对脓毒症患者凝血功能及炎性因子的影响。方法收集天津市津南医院2016年5月1日-2018年7月1日重症医学科收治的脓毒症患者75例,将其随机分为常规组、乌司他丁组和联合组(乌司他丁+低分子肝素)各25例,常规组患者给予常规治疗,乌司他丁组患者在常规组基础上给予乌司他丁治疗,联合组患者在乌司他丁组基础上,给予低分子肝素治疗。观察各组患者在治疗前与治疗后72h的血清炎性因子及凝血功能变化情况。结果治疗后72h与治疗前比较,各组患者的凝血功能相关指标及血清相关炎性因子水平均有所改善;与常规组比较,乌司他丁组、联合组得到明显改善,且联合组更加明显,差异有统计学意义(P<0.05)。结论乌司他丁与低分子肝素联合应用,可对患者的凝血功能起到改善作用,亦能有效降低炎性反应,值得临床推广应用。
Objective To observe the effect of Ulinastatin and low molecular weight heparin(LMWH)on coagulation function and inflammatory factors of the patients with sepsis.Methods From May 1,2016 to July 1,2018,75 patients with sepsis admitted to Intensive Care Unit in Tianjin Jinnan Hospital were collected and randomly divided into routine group,Ulinastatin group and combined group(Ulinastatin+LMWH)with 25 cases in each group.The patients in the routine group were given routine treatment,the patients in the Ulinastatin group were given Ulinastatin treatment on the basis of the routine group,the patients in the combined group were given LMWH treatment on the basis of the Ulinastatin group.The changes of serum inflammatory factors and coagulation function were observed before and 72 hours after treatment.Results Compared with those before treatment,the levels of indexes related to coagulation function and serum related inflammatory factors were improved in each group at 72 hours after treatment;compared with the routine group,Ulinastatin group and combined group were significantly improved,and the combined group was more obvious;the differences above all were statistically significant(P<0.05).Conclusion The combination application of Ulinastatin and LMWH could not only improve the coagulation function of patients,but also effectively reduce the inflammatory reaction,which was worthy of clinical popularization and application.
