阳和汤加减联合西药对骨结核患者外周血炎症因子及T淋巴细胞亚群的影响

Effect of Yanghe Decoction Combined with Western Medicine on Peripheral Inflammatory Factors and T Lymphocyte Subsets in Patients with Bone Tuberculosis

目的:探讨阳和汤加减联合西药对骨结核患者炎症因子及T淋巴细胞亚群的影响。方法:选取2014年6月~2017年6月我院收治的84例骨结核患者作为研究对象,按照随机数字表法将患者分为对照组(n=42)和治疗组(n=42);对照组应用西药治疗,治疗组在对照组治疗基础上联用阳和汤加减治疗。观察比较两组患者的相关炎症因子、T淋巴细胞亚群、临床症状恢复时间以及治疗效果。结果:与治疗前比较,治疗后两组的相关炎症因子水平、T淋巴细胞亚群水平均明显改善(P<0.05)。治疗后,与对照组比较,治疗组的肿瘤坏死因子α(TNF-α)、白细胞介素-6(IL-6)、C反应蛋白(CRP)、血沉(ESR)水平均明显降低,CD8^+水平明显降低,CD3^+、CD4^+、CD4^+/CD8^+水平均明显增高,治疗组的发热消失时间、脓肿吸收时间、血沉恢复时间、骨密度恢复时间均明显缩短,治疗总有效率(95.24%)明显高于对照组(76.19%)(P<0.05)。结论:与单用西药治疗骨结核比较,阳和汤加减联合西药对骨结核患者的治疗效果显著,能明显抑制患者炎症因子产生,增强患者的免疫功能,缩短其临床症状恢复时间,值得临床应用。

Objective:To investigate the effect of Yanghe decoction combined with western medicine on inflammatory factors and T lymphocytes subsets in patients with bone tuberculosis.Methods:84 patients with bone tuberculosis treated in our hospital from June 2014 to June 2017 were selected in the study.According to the random digital table,the patients were divided into the control group(n=42)and the treatment group(n=42).Patients in the control group were treated with western medicine,while patients in the treatment group were treated with Yanghe decoction on the basis of the same treatment used in the control group.The related inflammatory factors,T lymphocytes subsets,the recovery time of clinical symptoms and therapeutic effect were observed and compared between the two groups.Results:Compared with results before treatment,the levels of inflammatory factors and T lymphocyte subsets in the two groups were improved after treatment(P>0.05).After treatment,compared with the control group,the levels of tumor necrosis factorα(TNF-α)and IL-6,C reactive protein(CRP),erythrocyte sedimentation rate(ESR)in the treatment group were significantly decreased.The changes of T lymphocyte subsets in the treatment group were decreased,the levels of CD3^+,CD4^+and CD4^+/CD8^+were significantly increased.The recovery time of clinical symptoms in the treatment group was reduced,including fever disappearance time,abscess absorption time,erythrocyte sedimentation rate recovery time and bone mineral density recovery time.The total effective rate in the treatment group(95.24%)was significantly higher than that in the control group(76.19%)(P<0.05).Conclusion:Compared with western medicine,Yanghe decoction combined with western medicine is more effective in the treatment of bone tuberculosis,which can obviously inhibit the level of inflammatory factors,enhance the immune function of patients and shorten the recovery time of clinical symptoms,which is worthy of clinical application.
