老年住院患者静脉血栓栓塞症防控管理实施效果评价

Applied Effectiveness Evaluation of Prevention and Control of Venous Thromboembolism in Elderly Hospitalized Patients

目的评价某院老年住院患者实施静脉血栓栓塞症防控管理措施的应用效果。方法收集住院病案首页相关数据,统计2015年1月1日-2019年12月31日出院的老年住院患者VTE患病率和死亡率,比较VTE防控管理措施实施前1年和实施后5年各年度老年住院患者VTE患病率和死亡率的变化。结果实施干预措施后,老年住院患者VTE患病率出现短暂上升,由2015年的1.95%上升至2016年的2.24%,随后呈下降趋势,2019年降至0.54%。经卡方检验两两比较,2016年医院开展VTE防治管理工作后,老年住院患者VTE患病率逐年下降差异具有统计学意义(P<0.05)。老年住院患者VTE死亡率由2015年的0.06%上升至2016年的0.08%,随后呈下降趋势,2019年降至0.02%。经卡方检验两两比较,差异无统计学意义。结论院内VTE防控管理工作可以有效降低老年住院患者VTE患病率。同时要加强对患者的健康宣教,依托信息系统加强监管,提高医院VTE防治管理水平,保障医疗安全。

Objective To evaluate the effectiveness of the prevention and control measures of venous thromboembolism in the elderly hospitalized patients in hospital.Methods Through the collection of relevant data on the home page of inpatient medical records,the prevalence and mortality of VTE in elderly inpatients discharged from the hospital from 2015 to 2019 were counted,and the changes of the prevalence and mortality of VTE in elderly inpatients in each year before and 5 years after the implementation of VTE prevention and control management measures were compared.Results After the intervention,the morbidity rate of VTE in elderly hospitalized patients increased briefly,from 1.95%in 2015 to 2.24%in 2016,and then decreased to 0.54%in 2019.According to Chi-square test,after the hospital carried out VTE prevention and management work in 2016,the prevalence of VTE in elderly inpatients decreased year by year with statistical significance(P<0.05).The VTE mortality rate of elderly inpatients increased from 0.06%in 2015 to 0.08%in 2016,then decreased to 0.02%in 2019.There was no significant difference between the two groups by Chi-square test.Conclusions VTE prevention and control management could effectively reduce the prevalence of VTE in elderly inpatients.At the same time,we should strengthen the health education of patients,rely on the information system to strengthen the supervision,improve the VTE prevention and management ability,and ensure the medical safety.
