维持性血液透析患者家庭照顾者的希望水平现状及其影响因素分析

Analysis on status and influencing factors of hope level among family caregivers of maintenance hemodialysis patients

目的了解成都市某三甲中医院维持性血液透析(MHD)患者家庭照顾者的希望水平现状,分析其影响因素。方法采用便利抽样法,选取2018年6-10月在成都市某三甲中医院接受维持性血液透析治疗的203例患者及其相对应的家庭照顾者为调查对象,使用Herth希望指数量表(HHI)评估家庭照顾者的希望水平,采用自制调查问卷对维持性血液透析患者及其照顾者进行一般情况、疾病情况及照顾相关情况调查,并对数据进行分析。结果MHD患者家庭照顾者的希望水平得分范围为24~41,希望水平总分为33.38±4.06,其中68.47%(139例)的家庭照顾者为中等希望水平者;31.53%(64例)的家庭照顾者为高希望水平者。经多重线性回归显示,MHD患者家庭照顾者的年龄、教育程度、家庭人均月收入、照顾累计时长、参与照顾情况及相应患者的教育程度是其希望水平的主要影响因素(均P<0.05)。结论MHD患者家庭照顾者经历中等至高等希望水平,医务人员应重视MHD患者家庭照顾者的希望水平及其影响因素,积极采取针对性措施提高其希望水平,改善生活质量。

Objective To understand the current situation of hope level of family caregivers of maintenance hemodialysis(MHD)patients in a grade-A tertiary Traditional Chinese Medicine hospital in Chengdu City,and analyze its influencing factors.Methods Using convenient sampling method,203 patients receiving MHD in a grade-A tertiary Traditional Chinese Medicine hospital in Chengdu City from June to October in 2018 and their corresponding family caregivers were selected as the research objects.Hope level of family caregivers was assessed by Herth Hope Index(HHI).A self-made questionnaire was used to investigate the general situation,disease situation and care-related situation of MHD patients and their caregivers.The data were analyzed.Results Hope level score of family caregivers of MHD patients ranged from 24-41,the total score of hope level was 33.38±4.06,and 68.47%(139 cases)of family caregivers were those with middle hope level,while 31.53%(64 cases)were those with high hope level.Multiple linear regression showed that age,education level,family income per capita,cumulative length of care,participation in care of family caregivers,as well as education level of the corresponding patients were the main influencing factors of the hope level of family caregivers(all P<0.05).Conclusion Family caregivers of MHD patients experience moderate to high level of hope.Medical staff should attach importance to the level of hope of family caregivers of MHD patients and its influencing factors,and actively take targeted measures to improve their level of hope,so as to improve the quality of life.
