外用复合卵黄抗体在浅Ⅱ度烧伤创面愈合过程中抗感染作用的初步临床观察 被引量:2

摘要 目的:初步研究复合卵黄抗体喷剂对浅Ⅱ度烧伤创面愈合的作用。方法:将2018年7月5日—8月25日门诊收治的54例浅Ⅱ度烧伤的新发烧伤患者作为研究对象(其中14例患者为不同部位2处创面)。按随机数字表法分为两组,观察组34处创面予以碘伏消毒后外用复合卵黄抗体喷剂换药治疗;对照组34处创面予以常规碘伏换药治疗。创面每1~2 d换药1次,观察、记录创面大体情况及愈合时间;用药前、治疗第3、7、14 d及愈合后,创面部位采集分泌物标本进行细菌培养。结果:①10处创面因患者原因脱落,观察组2处创面出现过敏反应而停止试验用药,最终有效观察56处创面(46例患者),观察组23处(19例),对照组33处(27例)。②观察组、对照组创面用药前、用药后(3±1)d和用药后(7±1)d,两组细菌培养阳性率差异无统计学意义(P>0.05);愈合后1~2 d细菌培养阳性率分别为52.17%(12/23例)和78.79%(26/33例),差异具有统计学意义(P=0.036)。③观察组创面愈合时间为9(7,11)d,较对照组创面愈合时间12(9,13)d明显缩短(P=0.001)。④观察组创面2例出现过敏反应,对照组创面未见不良反应,但两组差异无统计学意义(8%比0,P=0.181)。结论:复合卵黄抗体喷剂用于创面抑菌具有与碘伏相似的效果,但可明显缩短愈合时间,并能降低浅II度烧伤创面愈合后细菌培养阳性率。 Objective:To preliminarily investigate the anti-infection effect of external compound yolk antibodies in superficial second degree burn wound healing.Methods:Fifty-four patients with 68 superficial second degree burn wounds,recruited in present study,were admitted in our Outpatient Department from Jul.5,to Aug.25,2018.Of them,14 patients had two wounds in different body regions.The wounds were randomly divided into control group and observation group(34 wounds each).Wounds in control group were treated with iodophor,and in observation group with compound yolk antibodies after iodophor disinfection.Changing the dressing every 1 or 2 days,and the general condition and healing time of each wound were simultaneously observed and recorded.Wound secretions were cultured before treatment,3rd,7th and 14th day after admission and after healing.Results:①Totally 10 wounds fell off due to the patients,and allergic reaction occurred in 2 wounds in observation group leading to the stop of trial,and sequentially 56 wounds(46 patients)were continuously observed(23 wounds of 19 cases in observation group and 33 wounds of 27 cases in control group).②There was no statistical significance in positive rates of bacterial culture between observation group and control group prior to treatment,(3±1)days later and(7±1)days later(P>0.05).But 1-2 days after total healing,the positive rates of bacterial culture in observation group and control group were 52.17%(12/23 cases)and 78.79%(26/33 cases),the difference was statistically significant(P=0.036).③After treatment,the healing time of wounds in observation group was 9(7,11)days,significantly shorter than that in control group[12(9,13)days,P=0.001].④During treatment,allergic reaction occurred in 2 cases of observation group,while no adverse reactions happened in control group,but the difference was not statistically significant(8%vs.0,P=0.181).Conclusions:Compound yolk antibodies spray possesses the similar anti-infection effect of iodophor,and may obviously shorten the healing time of superficial second degree burn wound,as well as decrease the positive rate of bacterial culture after healing.
