卡托普利治疗高血压合并急性肺损伤的疗效及对预后影响

Curative effect of captopril on hypertension combined with acute lung injury and its influences on prognosis

目的:探讨卡托普利治疗高血压合并急性肺损伤(ALI)患者的临床疗效及对预后影响情况。方法:选取就诊于某院的高血压合并急性肺损伤患者103例,将其随机分为观察组(52例)与对照组(51例)。观察组采用卡托普利片治疗,对照组给予硝苯地平缓释片治疗。2组均连续治疗7 d,比较2组患者治疗有效率、治疗前后呼吸频率、氧合指数(OI)、急性生理与慢性健康评分(APACHEII)、机械通气时间、重症加强护理病房入住时间、急性呼吸窘迫综合征(ARDS)的发病率、病死率及治疗期间不良反应发生率。结果:治疗7d后,观察组治疗总有效率高于对照组(P<0.05);治疗后,观察组OI高于对照组,呼吸频率、APACHEII低于对照组(P<0.05);观察组机械通气时间、重症加强护理病房入住时间及ARDS的发病率和病死率均低于对照组(P<0.05);2组患者不良反应发生率比较差异无统计学意义(P>0.05)。结论:与硝苯地平比较,卡托普利用于治疗高血压合并ALI疗效更好,能明显改善患者预后,且安全稳定。

Objective:To explore clinical curative effect of captopril on patients with hypertension combined with acute lung injury(ALI)and its influences on prognosis.Methods:A total of 103 patients with hypertension and ALI were randomly divided into an observation group(52 cases)and a control group(51 cases).The observation group was treated with captopril tablets,while the control group was given nifedipine sustained-release tablets.After continuous treatment of 7d,the two groups were compared in terms of the response rate of treatment,respiratory rate,oxygenation indexes(OI)and scores of acute physiology and chronic health evaluation(APACHE II)before and after the treatment,mechanical ventilation time,staying time in ICU,incidence and mortality of acute respiratory distress syndrome(ARDS)after the treatment,and incidence of adverse reactions during the treatment.Results:7d after the treatment,the total response rate of treatment in the observation group was significantly higher than that in the control group(P<0.05).After the treatmert OI of the observation group was higher than that of the control group(P<0.05),while respiratory rate,APACHE II were significantly lower than those in the control group(P<0.05).Mechanical ventilation time,staying time in ICU,and incidence and mortality of ARDS were significantly lower or shorter than those in the control group(P<0.05).There was no significant difference in incidence of adverse reactions between the two groups(P>0.05).Conclusion:Compared with nifedipine,the curative effect of captopril is better on hypertension combined with ALI,which can significantly improve prognosis of patients,with higher safety.
