通痹益脑汤对脑血栓患者神经功能及血液流变学的影响

Effects of Tongbi Yinao decoction on neurological function and hemorheology in patients with cerebral thrombosis

目的:探讨通痹益脑汤对脑血栓患者神经功能及血液流变学的影响。方法:选取脑血栓患者70例,按照随机数表法分为研究组与对照组,各35例。对照组采用常规治疗,研究组在对照组基础上联合通痹益脑汤治疗。比较治疗前及治疗2个月后,2组患者中医症状积分、美国国立卫生院神经功能缺损评分(NIHSS)、日常生活活动能力(Barthel指数)、血液流变学相关指标及血脂指标水平变化。结果:治疗2个月后,研究组患者头重如裹、呕恶痰涎、肢麻沉重各项中医症状积分、NIHSS评分、胆固醇、甘油三酯、低密度脂蛋白C水平均明显低于对照组(P<0.05),Barthel指数评分高于对照组(P<0.05);2组患者血浆黏度、全血低切黏度、全血高切黏度水平均较治疗前显著降低(P<0.05),且研究组明显低于同一时间对照组(P<0.05)。结论:通痹益脑汤可明显改善脑血栓患者疾病症状与神经功能,纠正患者血液流变学及血脂异常状态,提高患者日常生活能力。

Objective:To explore the effects of Tongbi Yinao decoction on neurological function and hemorheology in patients with cerebral thrombosis.Methods:70 patients with cerebral thrombosis were selected and randomly divided into an observation group and a control group,with 35 cases in each.The patients in the control group were given conventional treatment while those in the observation group were treated with Tongbi Yinao decoction on the basis of the control group.Before the treatment and 2 months after the treatment,scores of TCM symptoms,neurologic impairment scoring(NIHSS),activities of daily living(Barthel index),relevant indicators of hemorheology and blood lipid indicators were compared between the two groups.Results:2 months after the treatment,scores of TCM symptoms(heavy head,nausea and retching,and numb limbs),NIHSS scores and levels of CHOL,TG and LDL-C in the observation group were significantly decreased compared with those of the control group(P<0.05)and Barthel indexes in the observation group were significantly higher than those of the control group(P<0.05).Plasma viscosity,whole blood low-shear viscosity and whole blood high-shear viscosity in the two groups were significantly decreased compared with those before the treatment(P<0.05),with the indexes of the observation group being significantly lower than those of the control group at the same time point(P<0.05).Conclusion:Tongbi Yinao decoction can apparently improve the disease symptoms and neurological function of patients with cerebral thrombosis,correct their hemorheology and abnormal blood lipid status,and improve their activities of daily living.
