重组生长激素应用于卵巢正常反应而胚胎质量差患者的疗效研究

Efficacy analysis of recombinant growth hormone combined with controlled ovulation in the treatment of patients with normal ovarian response and low-quality embryo

目的探讨重组生长激素对前次体外受精/卵胞浆内单精子注射助孕失败的卵巢反应正常患者的临床结局的影响。方法选择在清远市人民医院生殖中心2017年7月至2018年7月行体外受精联合胚胎移植技术助孕治疗失败(胚胎质量差或无可用胚胎)而再次助孕治疗的患者42例作为研究对象。第二周期控制性超排卵前给予重组生长激素GH(4单位/日),皮下注射4周。观察比较前后两次促排卵后HCG日>14 mm卵泡数、获卵数、优质胚胎率及临床妊娠率等指标。结果应用重组生长激素联合控制性超排卵后,患者HCG日>14 mm卵泡数及获卵数明显增加,优质胚胎率及临床妊娠率均显著提高,差异具有统计学意义(P<0.05)。结论对于前次体外受精联合胚胎移植技术/卵胞浆内单精子显微注射技术助孕失败的正常卵巢反应的患者,应用重组生长激素联合控制性超排卵可有效改善卵母细胞、胚胎情况,能显著改善临床结局。

Objective To investigate the effect of recombinant growth hormone on the clinical outcome of patients with normal ovarian response who failed in previous in vitro fertilization/intracytoplasmic sperm injection.Methods 42 patients who underwent IVF/ICSI in the Qingyuan Reproductive Medicine center and received re-pregnancy treatment because of negative embryos or poor embryos quality form July 2017 to July 2018 were enrolled.Recombinant growth hormone GH(4 units/day)was injected subcutaneously for 4 weeks before the second cycle of controlled ovarian hyper-stimulation.The follicle number>14 mm on HCG day,the number of follicle obtained,the rate of high quality embryo and clinical pregnancy rate after ovulation induction were analyzed.Results After using the recombinant growth hormone,the number of mature follicles was significantly increased,and the rate of high-quality embryos and clinical pregnancy rate were significantly increased.The differences were statistically significant(all P<0.05).Conclusions Using recombinant growth hormone combined with controlled ovulation on patients with normal ovarian response who failed in previous IVF/ICSI,can effectively improve ovulation,quality of embryos and significantly improve clinical pregnancy rate.
