Er,Cr:YSGG激光治疗种植体周围炎的疗效评价及对炎症因子表达的影响

Effect of Er,Cr:YSGG laser on the treatment of peri-implantitis and the expression of inflammatory factors

目的探讨IL-6、TNF-α、MMP-8与种植体周围炎的相关性以及Er,Cr:YSGG激光对种植体周围炎的疗效。方法选择行种植体维护、符合标准的38名患者作为研究对象,以是否发生种植体周围炎将其分为炎症组和健康组,采集2组种植体周围龈沟液,ELISA法检测其中IL-6、TNF-α、MMP-8水平。炎症组采用Er,Cr:YSGG激光治疗,治疗后8周再次检测上述炎症因子水平。同时比较炎症组治疗前、炎症组治疗后以及健康组的牙周指标,包括菌斑指数(mPLI)、出血指数(mSBI)、探诊深度(PD)、临床附着水平(AL)。结果 Er,Cr:YSGG激光治疗种植体周围炎后,炎症组龈沟液中IL-6、TNF-α、MMP-8三种炎症因子水平较治疗前显著下降(P<0.05),mPLI、mSBI、PD、AL四项牙周指标也有明显改善(P<0.05),水平与健康组接近。结论 IL-6、TNF-α、MMP-8的表达与种植体周围炎密切相关,Er,Cr:YSGG激光可有效治疗种植体周围炎。

Objective To investigate the correlation between IL-6,TNF-α,MMP-8 and peri-implant inflammation and to learn the effect of Er,Cr:YSGG Laser on peri-implant inflammation.Methods 38 patients who underwent implant maintenance and met the standards were selected as the study objects.The patients were divided into inflammatory group and healthy group according to whether peri-implant inflammation occurred.Gingival crevicular fluid was collected and the levels of IL-6,TNF-αand MMP-8 were detected by ELISA.The inflammatory group was treated with Er,Cr:YSGG laser,and the inflammatory factors were detected again 8 weeks after treatment.At the same time,the periodontal indexes including plaque index(mPLI),bleeding index(mSBI),depth of exploration(PD)and clinical attachment level(AL)of inflammation group before treatment,after treatment and healthy group were compared.Results The levels of IL-6,TNF-αand MMP-8 in the inflammatory group were significantly lower than those before treatment(P<0.05);The periodontal indexes of mPLI,mSBI,PD and Al also improved significantly(P<0.05).Conclusion The expression of IL-6,TNF-αand MMP-8 are closely related to peri-implant inflammation.Er,Cr:YSGG laser can effectively cure peri-implant inflammation.
