左侧乳腺癌保乳术后两种调强放疗计划的剂量学对比

Dosimetric Comparison of Two Plans of Intensity Modulated Radiation Therapy After Breast Conserving Surgery for Left Breast Cancer

目的对比左侧乳腺癌保乳术后两种调强放疗计划的剂量学。方法 100例左侧乳腺癌保乳术患者随机分为两组各50例, A组采用FIF-F-IMRT计划, B组采用I-IMRT计划,比较两组患者的靶区剂量分布、心脏受照剂量、左肺受照剂量。结果靶区剂量方面, B组的Dmean显著低于A组(P <0.05)。心脏受照剂量方面, B组放疗后的V5、 V20、 V30、 Dmean均显著高于A组(P <0.05)。左肺受照剂量方面, B组放疗后的V5显著高于A组(P <0.05)。结论左侧乳腺癌保乳术后采用FIF-F-IMRT计划,可获得较好的靶区剂量分布,减少危及器官剂量。

Objective To compare the dosimetry of two plans of intensity modulated radiation therapy after breast conserving surgery for left breast cancer.Methods 100 patients with breast conserving surgery for left breast cancer were randomly divided into two groups,with 50 cases in each group.Group A received FIF-F-IMRT plan,while group B received I-IMRT plan.The target dose distribution,exposure dose of heart,and exposure dose of left pulmonary were compared between two groups.Results In terms of target dose,Dmean of group B was significantly lower than that of group A(P<0.05).In terms of exposure dose of heart,V5,V20,V30 and Dmean of group B after radiotherapy were significantly higher than those of group A(P<0.05).In terms of exposure dose of left pulmonary,V5 of group B after radiotherapy was significantly higher than that of group A(P<0.05).Conclusions The application of FIF-F-IMRT plan after breast conserving surgery for left breast cancer can obtain good target dose distribution and reduce the dose to organs at risk.
