摘要
目的探讨MCV、 MCH联合RDW检测在妊娠期地中海贫血筛查中的应用。方法选取2019年1月至2019年10月于我中心进行血液检测的7 000例疑似地中海贫血患者,均予以MCV、 MCH、 RDW检测及基因检测,分析MCV、 MCH、RDW单独与联合检测诊断妊娠期地中海贫血的结果。结果 7 000例疑似地中海贫血患者中, 29例经基因检测确诊为妊娠期地中海贫血。MCV、 MCH、 RDW联合检测的准确度、灵敏度、特异度、阳性预测值、阴性预测值分别为8.65%、 86.21%、 0.71%、8.17%、 33.33%;MCV、 MCH联合RDW检测诊断妊娠期地中海贫血的准确度、灵敏度、检测阳性预测值、阴性预测值均显著高于各指标单独检测(P <0.05)。结论 MCV、 MCH联合RDW检测对妊娠期地中海贫血具有较高的诊断价值。
Objective To explore the application of MCV and MCH combined with RDW detection in the screening of pregnancy thalassemia.Methods 7000 patients with suspected thalassemia who received blood test in our center from January 2019 to October 2019 were selected.All patients received MCV,MCH and RDWdetection and genetic test.The results of single and combined detection of MCV,MCH and RDW in the diagnosis of pregnancy thalassemia were analyzed.Results Of the 7000 patients with suspected thalassemias,29 cases were confirmed by genetic test to be pregnancy thalassemia.The accuracy,sensitivity,specificity,positive predictive value and negative predictive value of combined detection ofMCV,MCH and RDWwere 8.65%,86.21%,0.71%,8.17%and 33.33%,respectively.The accuracy,sensitivity,positive predictive value and negative predictive value of MCV and MCH combined with RDW detection in the diagnosis of pregnancy thalassaemia were significantly higher than those of single detection of MCV,MCH and RDW(P<0.05).Conclusions The combined detection of MCV,MCH and RDWhas high diagnostic value for pregnancy thalassemia.
