冠心病心力衰竭患者同型半胱氨酸及心率变异性相关性研究

Correlation between homocysteine and heart rate variability in patients with coronary heart failure

目的探讨冠心病心力衰竭患者血清同型半胱氨酸水平及心率变异性的相关性,为临床冠心病心力衰竭处理提供诊疗及预后判断的辅查实验依据。方法收集该科2017年1月至2018年12月入院的160例患者,根据病情分为试验组与对照组,试验组为120例冠心病心力衰竭患者,根据NYHA心功能分为3组(NYHAⅡ级、Ⅲ级、Ⅳ级),同时收集40例非冠心病患者为对照组。对所有患者均空腹查血清同型半胱氨酸水平、NT-ProBNP、血脂、血糖、及24小时动态心电图、心脏超声等检查,比较各组之间同型半胱氨酸水平、NT-ProBNP及心率变异性各项指标。结果心力衰竭患者同型半胱氨酸水平明显高于对照组,心率变异性低于对照组(P<0.05)。随着心功能分级的增加,其同型半胱氨酸水平增高,心率变异性降低。结论冠心病心力衰竭患者同型半胱氨酸水平高,其心率变异性降低,两者变化对冠心病心功能变化有重要意义。

Objective To investigate and explore the correlation between serum homocysteine level of Hcy and heart rate variability in patients with coronary heart failure for providing laboratory support as the basis of diagnosis and treatment of clinical coronary heart failure.Methods A total of 160 patients with coronary heart failure from January 2017 to December 2018 in our hospital were selected as the observation group.According to NYHA cardiac function,they were divided into three groups(NYHA class II,III and IV),and 40 patients with non-coronary heart disease were collected as the control group at the same time.All patients were checked for serum homocysteine levels of Hcy,NT-ProBNP,lipids and blood glucose on a fasting basis and 24 h dynamic ECG detection electrocardiograms,echocardiography.And homocysteine levels of Hcy,NT-ProBNP and heart rate variability indicators were compared between groups.Results The level of homocysteine of Hcy was significantly higher in patients with heart failure than in the control group,and the heart rate variability was lower than that in the control group(p<0.05).With the increase of cardiac function grades,homocysteine of Hcy levels increase,and heart rate variability decreases.Conclusion Patients with coronary heart failure have high levels of homocysteine of Hcy,and their heart rate variability is reduced.These two changes are of great significance for changes in coronary heart disease.
