彩色多普勒超声在妇科急腹症诊断中的应用探讨

Application of Color Doppler Ultrasound in Diagnosis of Acute Abdomen in Gynecology

目的:探讨临床治疗妇科急腹症采用彩色多普勒超声检查对诊断鉴别的作用。方法:选取本院2017年3月~2019年2月收治的82例妇科急腹症患者为研究对象。诊断时患者行阴道或腹部彩色多普勒超声检查,对比病理检查结果,统计本研究中诊断的符合率。结果:联合超声诊断符合率为95.12%,显著高于经阴道超声诊断符合率90.24%和腹部超声诊断符合率86.59%,P<0.05,差异具有统计学意义。经病理诊断结果发现82例妇科急腹症患者中有37例为异位妊娠、23例为先兆流产、11例为黄体囊肿破裂、8例为子宫内膜异位、3例为卵巢囊肿蒂扭转,其中联合超声诊断异位妊娠符合率为91.89%(34/37)、先兆流产符合率为86.96%(20/23)、黄体囊肿破裂符合率为81.81%(9/11)、子宫内膜异位符合率为100.00%(8/8)、卵巢囊肿蒂扭转符合率为100.00%(3/3)。结论:彩色多普勒超声在检查和诊断妇科急腹症中诊断符合率高,具有良好的应用价值。

Objective:To explore the role of color Doppler ultrasound in diagnosis and differentiation of clinical treatment of gynecological acute abdomen.Methods:A total of 82 patients with acute abdominal gynecology admitted to our hospital from March 2017 to February 2019 were selected as the research subjects.At the time of diagnosis,the patient underwent color Doppler ultrasonography of the vagina or abdomen.The pathological examination results were compared and the coincidence rate of diagnosis in this study was counted.Results:The coincidence rate of combined ultrasound diagnosis was 95.12%,which was significantly higher than that of transvaginal ultrasound diagnosis 90.24%and abdominal ultrasound diagnosis 86.59%,P<0.05,the difference was statistically significant.According to the pathological diagnosis,37 of 82 patients with gynecological acute abdomen had ectopic pregnancy,23 had threatened abortion,11 had corpus luteum cyst rupture,8 had endometriosis,and 3 had ovarian cyst torsion Among them,the coincidence rate of combined ultrasound diagnosis of ectopic pregnancy was 91.89%(34/37),the coincidence rate of threatened abortion was 86.96%(20/23),the coincidence rate of rupture of corpus luteum cyst was 81.81%(9/11),the coincidence rate of endometriosis is 100.00%(8/8),and the coincidence rate of ovarian cyst torsion was 100.00%(3/3).Conclusion:Color Doppler ultrasound has a high coincidence rate in the diagnosis and diagnosis of gynecological acute abdomen,and has good application value.
