心电图在急性肺栓塞诊断的作用探讨

The Role of Electrocardiogram in the Diagnosis of Acute Pulmonary Embolism

目的:深入探讨心电图在急性肺栓塞诊断中的作用效果。方法:从2017年12月~2018年12月本院收治的疑似急性肺栓塞患者中随机选取40例患者作为研究对象,医护人员对40例患者的病情状况实施回顾性分析,并将患者在入院时、溶栓治疗后以及出院时的心电图变化趋势进行分析和比较。结果:对40例疑似急性肺栓塞患者进行心电图检查后发现,其中窦性心动过速的患者有21例、房性早搏患者有6例、心房颤动患者有2例、ST-T改变患者16例、典型SIQ Ⅲ T Ⅲ患者14例、V1-V2ST波倒置患者30例、V1-V3T波倒置患者32例。肺型P波患者11例。结论:在诊断急性肺栓塞疾病的过程中,该疾病在心电图的检查过程中没有出现非特异性的特征,导致医护人员很容易出现误诊的情况。因此为了提高急性肺栓塞的诊断率,需要将诊断内容与患者的临床表现相结合进行判断,并且详细的将患者入院时的心电图变化、溶栓治疗后的心电图变化与出院时的心电图变化进行比较分析。

Objective:To explore the role of electrocardiogram in the diagnosis of acute pulmonary embolism.Methods:From the suspected patients with acute pulmonary embolism admitted in our hospital from December 2017 to December 2018,40 patients were randomly selected as the research object.The medical staff conducted a retrospective analysis of the condition of the 40 patients.The analysis and comparison of the electrocardiogram changes of the patients on admission,after thrombolysis and discharge.Results:After electrocardiogram examination of 40 patients with suspected acute pulmonary embolism,21 patients with sinus tachycardia,6 patients with atrial premature beats,2 patients with atrial fibrillation,16 patients with ST-T changes,14 patients with typical SIQ Ⅲ T Ⅲ,30 patients with V1-V2ST wave inversion,32 patients with V1-V3T wave inversion.Eleven patients with pulmonary P wave.Conclusion:In the process of diagnosing acute pulmonary embolism,the disease did not show non-specific features during the electrocardiogram examination,which caused medical staff to be prone to misdiagnosis.Therefore,in order to improve the diagnosis rate of acute pulmonary embolism,it is necessary to combine the diagnosis with the clinical manifestation of the patient to make a judgment,and compare the changes in the electrocardiogram of the patient on admission,the electrocardiogram after the thrombolytic treatment,and the electrocardiogram on discharge analysis.
