摘要
目的探讨全髋关节置换手术对老年股骨颈骨折患者的临床效果。方法选取徐州医科大学附属医院2016年3月至2018年6月收治的84例老年股骨颈骨折患者,采用信封抽签的方式将患者分为对照组和观察组,每组42例。对照组采用人工股骨头置换术治疗,观察组采用全髋关节置换术治疗。比较2组患者的手术相关指标、髋关节功能治疗前后改善情况、并发症发生率。结果与对照组比较,观察组术后引流量更少(P<0.05)。2组手术用时、术中失血量、离床活动时间比较,差异均无统计学意义(P>0.05)。观察组患者并发症发生率明显低于对照组(P<0.05)。术前2组患者的Harris评分比较,差异无统计学意义(P>0.05);术后,2组患者不同阶段的Harris评分明显提升,且观察组提升幅度更大(P<0.05)。结论全髋关节置换术适用于老年股骨颈骨折患者,能减少术后引流量,降低手术并发症,促进髋关节功能逐渐恢复。
Objective To explore the clinical effect of total hip replacement surgery for elderly patients with femoral neck fractures.Methods Totally 84 elderly patients with femoral neck fractures treated in the Affiliated Hospital of Xuzhou Medical University from March 2016 to June 2018 were selected.The patients were divided into 2 groups by envelope drawing,and 42 patients in each group.The control group was treated with artificial femoral head replacement,and the observation group was treated with total hip replacement.The surgical-related indicators,improvement of hip function before and after treatment,and complications were compared between the two groups.Results Compared with the control group,the postoperative drainage volume in the observation group was less(P<0.05).There was no significant difference in the operation time,intraoperative blood loss,and activity time away from bed between the two groups(P>0.05).The incidence of complications in the observation group was significantly lower than that in the control group(P<0.05).There was no significant difference in Harris score between the two groups before surgery(P>0.05);the Harris scores of the two groups of patients at different stages increased significantly after surgery,and the improvement in the observation group was greater(P<0.05).Conclusion Total hip replacement surgery for elderly patients with femoral neck fractures can reduce postoperative drainage and surgical complications,which can gradually restore hip function.
