微创手术对脊柱创伤患者围术期指标、脊柱功能及术后并发症的影响 被引量:11

Effect of Minimally Invasive Surgery on Perioperative Indexes,Spinal Function and Postoperative Complications in Patients with Spinal Trauma

摘要 目的:探讨微创手术对脊柱创伤患者围术期指标、脊柱功能及术后并发症的影响。方法:选取脊柱创伤患者76例,采用随机数字表法均分为观察组和对照组;观察组实施微创手术、对照组采用传统开放手术,记录2组患者围术期指标(手术时间、术中出血量、手术切口长度、住院时间、下床活动时间、术后引流量)、手术治疗效果及手术前后损伤椎体科伯角度(Cobb’s角)、前缘高度、矢状位指数(SI)、McGill疼痛量表评分、Oswestry功能障碍指数评分(ODI)、Prolo量表评分及并发症发生率。结果:观察组手术时间、住院时间、下床活动时间、术中出血量、手术切口长度及术后引流量低于对照组,治疗有效率高于对照组,差异有统计学意义(P<0.05);观察组术后6个月,损伤椎体Cobb’s角低于对照组,而前缘高度、SI高于对照组,差异有统计学意义(P<0.05);观察组术后McGill评分、ODI评分低于对照组,Prolo评分高于对照组,差异有统计学意义(P<0.01);观察组术后6个月并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:微创手术可明显改善患者围术期指标、损伤椎体Cobb’s角、脊柱功能,降低疼痛度,且术后并发症少。 Objective:To investigate the effect of minimally invasive surgery(MIS)on perioperative indexes,spinal function and postoperative complications in patients with spinal trauma.Methods:Seventy-six patients with spinal trauma were randomly divided into the observation group and the control group.The observation group was given MIS,while the control group was given traditional open surgery.We recorded the perioperative indexes(operation time,the median bleeding volume,length of surgical incision,hospital stay,postoperative activity time,postoperative drainage),surgery effect,imaging data before and after surgery[Cobb's angle of injured vertebrae,anterior margin height,sagittal index(SI)],McGill pain scale questionnaires,Oswestry dysfunction index(ODI)score,Prolo scale score and complications.Results:MIS decreased the perioperative indexes,but improved surgery efficiency(P<0.05).At 6 months post-surgery,MIS reduced the Cobb's angle of the injured vertebral body,increased the anterior edge height and SI(P<0.05).Moreover,MIS decreased the scores of McGill pain scale and ODI scores,but increased the Prolo scale score(P<0.01).Furthermore,MIS reduced the incidence of complications at 6 months after the surgery(P<0.05).Conclusion:MIS significantly improvs spinal function by reducing patients'perioperative indexes,Cobb's angle of injured vertebral,pain scores and postoperative complications.
