住院老年癌症共病病人营养不良现状调查及影响因素分析

Investigation on malnutrition in elderly in patients with cancer comorbidity and its influencing factors

[目的]调查住院老年癌症共病病人的营养不良状况并分析其影响因素。[方法]采用便利抽样选取老年癌症共病病人316例,应用病人主观整体营养评估(PG⁃SGA)对病人进行营养评估,应用住院病人焦虑抑郁量表对病人进行心理状态评估,收集病人的一般资料、疾病相关资料、饮食相关资料和实验室检查结果。采用Logistic回归分析老年癌症共病病人营养不良的影响因素。[结果]316例老年癌症共病病人PG⁃SGA评分(6.51±5.26)分,其中47例(14.9%)病人营养状况良好,269例(85.1%)病人存在营养不良。回归分析结果显示,影响老年癌症共病病人营养状态的因素有癌症类型、家庭人均月收入、营养支持、治疗方式、白蛋白和抑郁情绪。[结论]老年癌症共病病人营养不良发生率高,医护人员应根据其影响因素采取有效的预防措施,降低老年癌症共病病人营养不良的发生率。

Objective:To investigate the malnutrition status of hospitalized elderly patients with cancer comorbidity and analyze its influencing factors.Methods:A total of 316 elderly patients with cancer comorbidity,were selected by convenient sampling.PG⁃SGA was used to evaluate the nutrition of the patients.And Hospital Anxiety and Depression Scale was used to evaluate the psychological status of the patients.The general data,disease⁃related data,diet⁃related data and laboratory results of the patients were collected.The influencing factors of malnutrition in elderly patients with cancer comorbidity were analyzed using logistic regression analysis.Results:The PG⁃SGA scores of 316 elderly patients with cancer comorbidity was 6.51±5.26.Among them,47 patients(14.9%)had good nutritional status.And 269 patients(85.1%)had malnutrition.The results of regression analysis showed that the factors influencing nutrition status of elderly patients with cancer comorbidity were cancer type,average monthly income of family,nutritional support,treatment method,albumin concentration and depression.Conclusion:The incidence of malnutrition in elderly patients with cancer comorbidity was high.Medical staff should take effective preventive measures according to its influencing factors,so as to reduce the incidence of malnutrition in elderly patients with cancer comorbidity.
