厚朴排气合剂在瘢痕子宫剖宫产术后的应用效果

Application of Houpu Paiqi Mixture following Cesarean Section in Women with Scarred Uterus

目的探讨厚朴排气合剂在瘢痕子宫剖宫产患者术后的应用效果。方法将93例瘢痕子宫剖宫产患者按随机数字表法分为2组:对照组46例术后给予维持水电解质酸碱平衡,常规对症抗感染治疗;观察组47例在对照组治疗基础上给予厚朴排气合剂口服治疗。比较2组初乳时间、血性恶露量、血性恶露持续时间、腹胀消失时间、肠鸣音恢复时间及肛门排气时间,观察2组干预前后血管活性肠肽(VIP)、血浆胃动素(MOT)水平变化情况。结果观察组治疗后VIP水平、腹胀消失时间、初乳时间、血性恶露持续时间、肠鸣音恢复时间、血性恶露量及肛门排气时间均显著低于对照组,MOT水平显著高于对照组,2组比较差异均有统计学意义(P<0.05)。结论厚朴排气合剂能够有效缩短剖宫产术后患者胃肠功能恢复时间及初乳时间,促进子宫复旧,其机制可能与调节MOT、VIP水平,增强子宫收缩有关。

Objective To evaluate the application of Houpu Paiqi mixture following cesarean section in women with scarred uterus.Methods A total of 93 patients with scarred uterus were randomly divided into control group(n=46)and observation group(n=47).Both groups were treated with postoperative maintenance of water,electrolyte and acid-base balance balance and routine symptomatic anti-infective therapy.Additionally,the observation group was orally given Houpu Paiqi mixture.Colostrum time,amount of bloody lochia,duration of bloody lochia,time to disappearance of abdominal distension,time to recovery of borborygmus and time to first anal flatus were compared between the two groups.In addition,levels of vasoactive intestinal peptide(VIP)and motilin(MOT)were measured before and after treatment.Results Compared with control group,Houpu Paiqi mixture decreased VIP level,colostrum time,duration of bloody lochia,amount of bloody lochia,time to disappearance of abdominal distension,time to recovery of borborygmus and time to first anal flatus and increased MOT level(P<0.05).Conclusion Houpu Paiqi mixture can effectively shorten the time of recovery of gastrointestinal function and colostrum time and promote uterine involution after cesarean section.The mechanisms may be involved in the regulation of MOT and VIP levels and uterine contraction.
