硫糖铝混悬液联合奥美拉唑肠溶片治疗隆起糜烂性胃炎

Efficacy of sucralfate suspension combined with omeprazole enteric-coated tablets on treatment of raised erosive gastritis

目的探讨硫糖铝混悬液联合奥美拉唑肠溶片治疗隆起糜烂性胃炎(REG)的疗效。方法选取2016年5月~2019年5月四川大学华西医院收治的REG患者82例,采取抽签法随机分为观察组和对照组,每组各41例。观察组患者予以硫糖铝混悬液联合奥美拉唑肠溶片治疗,对照组患者予以奥美拉唑肠溶片治疗,幽门螺杆菌(Hp)阳性患者予以抗生素进行根除Hp治疗。比较两组患者临床疗效、Hp转阴率的差异性,观察治疗前后血清胃蛋白酶原(PGⅠ、PGⅡ)、胃泌素-17(GAS-17)、炎症因子[白介素-8(IL-8)、IL-10、肿瘤坏死因子-α(TNF-α)]水平及胃黏膜组织中表皮生长因子(EGF)、碱性成纤维细胞生长因子(bFGF)、抗原Ki-67表达情况。结果观察组患者治疗总有效率高于对照组(P<0.05);两组患者Hp转阴率比较差异无统计学意义(P>0.05);治疗8周后,两组患者血清PGⅠ、GAS-17水平和PGR均较治疗前升高,且观察组高于对照组(P<0.05),血清PGⅡ、IL-8、IL-10、TNF-α水平较治疗前降低,且观察组低于对照组(P<0.05);治疗8周后,两组患者胃黏膜EGF、bFGF、Ki-67表达水平均较治疗前降低,且观察组低于对照组(P<0.05)。结论硫糖铝混悬液联合奥美拉唑肠溶片治疗REG具有良好的协同作用,可有效减轻炎症反应,促进胃黏膜功能恢复,有利于疾病转归。

Objective To explore the efficacy of sucralfate suspension combined with omeprazole enteric-coated tablets on treatment of raised erosive gastritis(REG).Methods 82 patients with REG admitted to West China Hospital,Sichuan University from May 2016 to May 2019 were selected and randomly divided into observation group and control group,according to the envelope method.Observation group was treated with sucralfate suspension+omeprazole enteric-coated tablets,and control group was treated with omeprazole enteric-coated tablets.The patients with positive helicobacter pylori(Hp)were treated with antibiotics for eradication of Hp.The clinical efficacy and Hp negative conversion rate were compared between the two groups.Serum pepsinogens(PGI,PGII),gastrin-17(GAS-17),inflammatory factors[interleukin-8(IL-8),IL-10,tumor necrosis factor-α(TNF-α)]and expressions of epidermal growth factor(EGF),basic fibroblast growth factor(bFGF)and antigen Ki-67 in gastric mucosal tissues were observed before and after treatment.Results The total effective rate of treatment in observation group was higher than that in control group(P<0.05).There was no significant difference in the Hp negative conversion rate between the two groups(P>0.05).After 8 weeks of treatment,the levels of serum PGI and GAS-17 and PGR in the two groups were higher than those before treatment.The levels in observation group were higher than those in control group(P<0.05).The levels of serum PGII,IL-8,IL-10 and TNF-αwere lower than those before treatment,and the levels in observation group were lower than those in control group(P<0.05).After 8 weeks of treatment,the levels of EGF,bFGF and Ki-67 in gastric mucosa in the two groups were lower than those before treatment,and the levels in observation group were lower than those in control group(P<0.05).Conclusion Sucralfate suspension combined with omeprazole enteric-coated tablets has a good synergistic effect on REG.It can effectively reduce the inflammatory response,promote the recovery of gastric mucosal function,and facilitate the outcome of the disease.
