文章介绍了英国、美国、德国和古巴等不同国家的家庭医生签约服务制度,对服务模式、激励机制和制度影响进行了总结归纳,为中国家庭医生签约制度提出借鉴。提出建立以家庭医生为核心的基层医疗服务制度是促进国家卫生体系完善的重要举措,要进一步形成家庭医生有效签约机制,需深化签约服务内涵建设;完善家庭医生与签约服务挂钩的激励机制,保障家庭医生的收入水平;开展家庭医生签约服务的考核管理,完善家庭医生签约服务制度。

This paper introduced the contracted family doctor services in the countries of the United Kingdom,the United States,Germany and Cuba,and summarized the service model,incentive mechanism and institutional impact to provide reference for the contracted family doctor services in China.It is an important measure to promote the national health system by establishing a primary medical service system with family doctors at its core.It is necessary to further develop an effective contracting mechanism for family doctors,deepen the connotation of contracted services,improve the incentive mechanism of linking family doctors and contracted services,and ensure the income of family doctors;meanwhile,it is also necessary to carry out assessment management of contracted family doctor services and improve contracted family doctor service system.
