摘要
目的探究弹性成像与高频彩超两种方法对甲状腺结节患者的临床诊断效果及准确率的影响。方法选取2017年1月~2018年8月在我院就诊的甲状腺结节患者。将其按照不同诊断方法分为两组,每组68例。高频彩超组(HI组):采用高频超声诊断甲状腺结节;超声引导弹性成像组(UG组):采用彩超联合弹性成像诊断。最终诊断由手术确诊或超声随访1年做出。结果高频彩超检测准确率较低,超声引导弹性成像检测较准确。HI组与组织病理学诊断结果比较,差异有统计学意义(P<0.05),UG组与组织病理学诊断结果比较,差异无统计学意义(P>0.05)。HI组和UG组比较,UG组检测诊断的特异性、准确率均高于HI组,差异有统计学意义(P<0.05),两组诊断甲状腺结节的灵敏度比较差异无统计学意义(P>0.05)。结论弹性成像与高频彩超两种诊断方法对甲状腺结节均有相应的临床诊断效果,但两种诊断方法相结合的诊断准确率更高,诊断价值更高。
Objective To explore the effects of high-frequency color Doppler ultrasound and elastography in the differential diagnosis of benign and malignant thyroid nodules and their accuracy.Methods Patients with thyroid nodules who were treated in our hospital from January 2017 to August 2018 were selected.All patients were divided into two groups according to different diagnostic methods,with 68 cases in each group.The high-frequency color Doppler ultrasound group(HI group)was diagnosed by high-frequency color Doppler ultrasound,while the ultrasoundguided elastography group(UG group)was diagnosed by color Doppler ultrasound and elastography.The final diagnosis was made by surgical confirmation or one-year follow-up with ultrasound.Results The accuracy of high-frequency color Doppler ultrasound detection was lower than that of ultrasound-guided elastography.There was difference in the diagnosis results between the HI group and histopathological diagnosis(P<0.05);and there was no difference in the diagnosis results between the UG group and histopathological diagnosis(P>0.05).The specificity and accuracy of the detection and diagnosis of the UG group were higher than those of the HI group,and the differences were statistically significant(P<0.05).There was no difference in the sensitivity of diagnosing thyroid nodules between the UG group and HI group(P>0.05).Conclusion The high-frequency color Doppler ultrasound and elastography have corresponding clinical diagnosis effects on thyroid nodules,but the combination of the two diagnostic methods has higher diagnostic accuracy and higher diagnostic value.
