血液灌流对同型半胱氨酸及心脑血管疾病的影响 被引量:1

摘要 目的通过研究血液灌流联合血液透析与单纯血液透析的疗效,了解血液灌流联合血液透析对同型半胱氨酸及心脑血管疾病的影响。方法研究对象为2017年5月~2018年5月于我院接受血液透析治疗的90例患者,分组方法为随机数字表法,每组45例。将血液灌流联合血液透析治疗应用于观察组治疗中,将单纯血液透析治疗应用于对照组治疗中。观察12个月后两组对同型半胱氨酸及心脑血管疾病的影响情况。结果观察12个月后,观察组患者的Hb、Alb及前白蛋白的值分别为(76.47±3.67)g/L、(36.54±2.76)g/L及(189.80±3.37)g/L,对照组患者的Hb、Alb及前白蛋白的值分别为(76.39±3.68)g/L、(36.70±2.87)g/L以及(189.77±3.35)g/L,差异无统计学意义(P>0.05);观察组患者的叶酸(14.29±1.78)nmol/L与对照组患者的叶酸(14.35±1.79)nmol/L比较,差异无统计学意义(P>0.05);观察组患者的Hcy(16.09±1.90)µmol/L显著低于对照组患者的Hcy(25.01±2.65)µmol/L,差异有统计学意义(P<0.05);观察组患者的CHOL(5.80±0.37)mmol/L、TG(6.61±1.00)mmol/L、Scr(833.21±71.99)µmol/L及BUN(18.54±3.22)mmol/L显著低于对照组患者的CHOL(6.38±0.42)mmol/L、TG(7.10±1.18)mmol/L、Scr(910.44±76.32)µmol/L及BUN(28.54±4.22)mmol/L,差异有统计学意义(P<0.05)。结论血液灌流联合血液透析对清除同型半胱氨酸更有效,更好的降低心脑血管疾病发生率。 Objective To understand the effect of hemoperfusion combined with hemodialysis on homocysteine(Hcy)and cardiovascular and cerebrovascular diseases by studying the effects of hemoperfusion combined with hemodialysis and hemodialysis alone.Methods A total of 90 patients who underwent hemodialysis in our hospital from May 2017 to May 2018 were selected the study subjects.They were divided into two groups using a random number table method,with 45 patients in each group.The observation group was given hemoperfusion combined with hemodialysis,and the control group was given hemodialysis alone.After 12 months,the effect of the two methods on Hcy and cardiovascular and cerebrovascular diseases was observed.Results After 12 months of observation,the values of hemoglobin(Hb),albumin(Alb)and prealbumin in the observation group were(76.47±3.67)g/L,(36.54±2.76)g/L,and(189.80±3.37)g/L respectively,and(76.39±3.68)g/L,(36.70±2.87)g/L,and(189.77±3.35)g/L,respectively in the control group,and the differences were not statistically significant(P>0.05);there was no difference in folic acid between the observation group and control group[(14.29±1.78)nmol/L vs(14.35±1.79)nmol/L](P>0.05);the Hcy level of the observation group was(16.09±1.90)µmol/L,which was significantly lower than(25.01±2.65)µmol/L of the control group,and the difference was statistically significant(P<0.05);the cholesterol(CHOL),triglyceride(TG),serum creatinine(Scr)and blood urea nitrogen(BUN)levels of the observation group were(5.80±0.37)mmol/L,(6.61±1.00)mmol/L,(833.21±71.99)µmol/L,and(18.54±3.22)mmol/L,respectively,which were significantly lower than those of the control group[CHOL(6.38±0.42)mmol/L,TG(7.10±1.18)mmol/L,Scr(910.44±76.32)µmol/L,and BUN(28.54±4.22)mmol/L],and the differences were statistically significant(P<0.05).Conclusion Hemoperfusion combined with hemodialysis is more effective in removing Hcy and can better reduce the incidence of cardiovascular and cerebrovascular diseases.
