微创切开引流术治疗乳腺脓肿效果以及对患者哺乳的影响

The effect of minimally invasive operation and incision drainage in the treatment of breast abscess and its influence on breastfeeding

目的分析乳腺脓肿微创手术治疗效果以及对哺乳的影响。方法将2017年3月~2018年3月在我院进行哺乳期乳腺脓肿传统开放手术治疗的47例患者作为常规组,我院自2018年4月开始对乳腺脓肿患者采用微创手术方式治疗,将2018年4月~2019年4月在我院进行乳腺脓肿微创手术治疗的47例患者作为实验组,回顾性分析患者临床资料,观察其手术后哺乳情况。比较各组产妇手术时间、手术切口长度、手术中出血量;换药次数、炎症控制时间、住院时间、疼痛程度评分、切口瘢痕长度、切口愈合时间;手术后并发症;疾病复发率以及母乳喂养率。结果实验组在手术时间、手术切口长度方面短于常规组,在手术中出血量方面低于常规组(P<0.05)。实验组换药次数少于常规组,炎症控制时间、住院时间、切口瘢痕长度以及切口愈合时间均短于常规组,疼痛评分低于常规组(P<0.05)。实验组产妇手术后在感觉障碍、手术后感染以及乳漏方面出现率均低于常规组(P<0.05)。实验组疾病复发率低于常规组,母乳喂养率高于常规组(P<0.05)。结论哺乳期乳腺脓肿患者采用微创手术方式治疗可缩短患者手术时间以及住院时间,降低手术切口长度、手术中出血量以及疾病复发率,从而改善患者疼痛情况,减少并发症出现率,促进患者伤口愈合,提升母乳喂养率,具有一定的临床应用价值。

Objective To analyze the effect of minimally invasive operation on breast abscess and its influence on breastfeeding.Methods 47 patients undergoing traditional open surgery for breast abscesses in our hospital from March 2017 to March 2018 were selected as the conventional group.Our hospital has adopted minimally invasive operation for breast abscess patients since April 2018.47 patients who underwent minimally invasive operation for breast abscess in our hospital from April 2018 to April 2019 were selected as the study group.The clinical data of the patients were retrospectively analyzed and the breastfeeding situation after the operation was observed.The maternal operation duration,surgical incision length,and intraoperative bleeding,number of dressing changes,inflammation control time,hospital stay,pain level,incision scar length,incision healing time,postoperative complications,disease recurrence rate and breastfeeding rate were compared between the two groups.Results The operation duration and surgical incision length in the study group were shorter than those in the conventional group,and the amount of intraoperative bleeding was less than that in the conventional group(P<0.05).The number of dressing changes in the study group was less than that in the conventional group,and the inflammation control time,hospital stay,incision scar length and the incision healing time were shorter than those in the conventional group,and the pain score was lower than that in the conventional group(P<0.05).The incidence of maternal sensory disturbance,postoperative infection and breast leakage in the study group was lower than that in the conventional group(P<0.05).The disease recurrence rate of the study group was lower than that of the conventional group,and the breastfeeding rate was higher than that of the conventional group(P<0.05).Conclusion The minimally invasive operation for breast abscess patients in lactation period can shorten the operation duration and hospital stay,reduce surgical incision length,the amount of intraoperative bleeding and the rate of disease recurrence,thus improving the patient’s pain situation and reducing the incidence of complications.It can also promote wound healing of patients and increase breastfeeding rate,which is worthy of clinical application.
