MRI联合扩散加权成像在宫颈癌分期诊断中的应用效果评价

Evaluation on the application effect of MRI combined with diffusion weighted imaging in the staging diagnosis of cervical cancer

目的探讨MRI联合扩散加权成像(DWI)在宫颈癌分期诊断中的应用效果。方法将2018年3月~2019年9月我院收治的65例宫颈癌患者作为宫颈癌组,选取同时期65名正常宫颈人群作为正常组,分别对其进行常规MRI平扫及扩散加权成像(DWI)联合动态增强磁共振成像(DCE-MRI)检测,对两种检查方式的准确率情况、正常宫颈及宫颈癌的MRI表现、DWI及ADC值、DCE-MRI各参数的分析情况进行观察。结果DCE-MRI结合DWI在宫颈癌分期中的准确率明显高于常规MRI诊断的准确率;正常子宫颈的ADC值明显高于宫颈癌ADC值;随着分级程度的不断升高,ADC值不断降低,差异有统计学意义(P<0.05);62例患者MRI增强扫描的时间-信号曲线表现为平台型,3例患者表现为流入型;DCE-MRI定量参数Ktrans、Ve、Kep值随着宫颈癌分期的升高而变大,差异有统计学意义(P<0.05)。结论MRI联合扩散加权成像在宫颈癌分期诊断中的应用具有显著的临床效果。

Objective To investigate the application effect of MRI combined with diffusion weighted imaging(DWI)in the staging diagnosis of cervical cancer.Methods 65 patients with cervical cancer admitted and treated in our hospital from March 2018 to September 2019 were selected as the cervical cancer group,and 65 normal cervical populations during the same period were selected as the normal group,which were detected with conventional MRI plain scan and diffusion weighted imaging(DWI)combined with dynamic enhanced magnetic resonance imaging(DCE-MRI),respectively.The accuracy of the two examination methods,MRI manifestations of normal cervix and cervical cancer,DWI and ADC values,and DCE-MRI parameters were observed.Results The accuracy of DCE-MRI combined with DWI in cervical cancer staging was significantly higher than that of conventional MRI;The ADC value of normal cervix was significantly higher than that of cervical cancer;With the increasing of the grading degree,the ADC value decreased continuously,and the difference was statistically significant(P<0.05);The time-signal curve of MRI enhanced scan in 62 patients showed platform-type,and that in 3 patients showed inflow-type;the quantitative parameters of DCE-MRI,i.e.,Ktrans,Ve and Kep values,increased with the increase of cervical cancer stage,and the difference was statistically significant(P<0.05).Conclusion The application of MRI combined with diffusion weighted imaging in the staging diagnosis of cervical cancer has a significant clinical effect.
