摘要
目的 探讨骨科三号方联合阿仑膦酸钠维D3片治疗骨质疏松性髋部骨折的临床效果.方法 120例骨质疏松性髋部骨折患者,按照随机数字表法分为对照组和治疗组,各60例.两组患者均予闭合复位防旋股骨近端髓内钉(PFNA)内固定术,对照组患者予阿仑膦酸钠维D3片治疗,治疗组患者在对照组基础上联合骨科三号方颗粒剂治疗.比较两组患者临床疗效;骨代谢指标[血清钙(S-Ca)、血清磷(S-P)、骨碱性磷酸酶(B-ALP)]及骨密度(BMD)水平;治疗前后视觉模拟评分法(VAS)评分和髋关节功能评分(Harris评分).结果 治疗组患者的总有效率96.67%明显高于对照组的86.67%,差异具有统计学意义(P<0.05).治疗后,两组患者的S-Ca、BMD水平均高于治疗前,S-P、B-ALP水平低于治疗前,差异均有统计学意义(P<0.05);治疗组患者的S-Ca、BMD水平均高于对照组,S-P、B-ALP水平均低于对照组,差异均具有统计学意义(P<0.05).治疗前,两组患者VAS评分和Harris评分比较差异无统计学意义(P>0.05);治疗后,治疗组患者的VAS评分(2.08±0.99)分明显低于对照组的(3.54±1.04)分,Harris评分(86.14±8.76)分明显高于对照组的(70.34±9.15)分,差异具有统计学意义(P<0.05).结论 骨科三号方联合阿仑膦酸钠维D3片治疗骨质疏松性髋部骨折的临床疗效显著,可明显改善患者的骨代谢和BMD,减少患者疼痛和改善髋关节功能.
Objective To discuss the clinical effect of No.3 orthopedic medicine combined with alendronate sodium and vitamin D3 tablet in the treatment of osteoporotic hip fracture.Methods A total of 120 patients with osteoporotic hip fracture were divided into control group and treatment group by random numerical table,with 60 cases in each group.Both groups received closed reduction and internal fixation with proximal femoral nail anti-rotation(PFNA).The control group was treated by alendronate sodium and vitamin D3 tablets,and the treatment group was treated by No.3 orthopedic granule on the basis of the control group.The clinical efficacy,bone metabolism markers[serum calcium(S-Ca),serum phosphorus(S-P),bone alkaline phosphatase(B-ALP)],bone mineral density(BMD),visual analogue scale(VAS)score and Harris score were compared between the two groups.Results The total effective rate 96.67%of the treatment group was obviously higher than that of the control group 86.67%,and the difference was statistically significant(P<0.05).After treatment,the levels of S-Ca and BMD of the two groups were higher than those before treatment,and levels of S-P and B-ALP were lower than those before treatment,and the difference was statistically significant(P<0.05).The levels of S-Ca and BMD of the treatment group were higher than those of the control group,and levels of S-P and B-ALP were lower than those of the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no statistically significant difference in VAS score and Harris score between the two groups(P>0.05).After treatment,VAS score(2.08±0.99)points of the treatment group was obviously lower than that of the control group(3.54±1.04)points and Harris score(86.14±8.76)was obviously higher than that of the control group(70.34±9.15)points,and the difference was statistically significant(P<0.05).Conclusion No.3 orthopedic medicine combined with alendronate sodium and vitamin D3 tablets shows remarkabel clinical efficacy in the treatment of osteoporotic hip fracture,which can significantly improve patients’bone metabolism and BMD,reduce patients’pain and improve hip function.
