摘要
目的 观察对慢性阻塞性肺疾病合并呼吸衰竭患者使用无创呼吸机通气治疗的效果.方法 100例慢性阻塞性肺疾病合并呼吸衰竭患者,采用随机数字表法分为对照组和试验组,每组50例.对照组患者采用常规支持治疗,试验组患者在对照组基础上联合无创呼吸机通气治疗.比较两组临床疗效及治疗前后血气指标、肺功能指标变化情况.结果 试验组患者总有效率为94.00%,高于对照组的78.00%,差异具有统计学意义(P<0.05).治疗前,两组患者二氧化碳分压(PaCO2)比较差异无统计学意义(P>0.05);治疗后,两组患者PaCO2均低于本组治疗前,且试验组(45.04±12.12)mm Hg(1 mm Hg=0.133 kPa)低于对照组的(61.54±11.93)mm Hg,差异具有统计学意义(P<0.05).治疗前,两组患者第1秒用力呼气容积(FEV1)、第1秒用力呼气容积占用力肺活量的百分比(FEV1/FVC)及第1秒用力呼气容积占预计值的百分比(FEV1%pred)比较,差异无统计学意义(P>0.05);治疗后,两组患者FEV1、FEV1/FVC及FEV1%pred均高于本组治疗前,且试验组高于对照组,差异具有统计学意义(P<0.05).结论 给予慢性阻塞性肺疾病合并呼吸衰竭患者无创呼吸机通气治疗疗效确切,可使患者血气指标及肺功能指标得到有效改善,值得在临床上推广使用.
Objective To observe the effect of noninvasive ventilator ventilation on patients with chronic obstructive pulmonary disease and respiratory failure.Methods A total of 100 patients with chronic obstructive pulmonary disease and respiratory failure were divided into control group and experimental group by random numerical table,with 50 cases in each group.Patients in the control group were treated with routine support,and patients in the experimental group were treated with noninvasive ventilator ventilation on the basis of the control group.The clinical efficacy,changes of blood gas indexes and pulmonary function indexes before and after treatment were compared between the two groups.Results The total effective rate of the experimental group was 94.00%,which was higher than that of the control group 78.00%,and the difference was statistically significant(P<0.05).Before treatment,there was no statistically significant difference in partial pressure of carbon dioxide(PaCO2)between the two groups(P>0.05).After treatment,PaCO2 of the two groups were lower than those before treatment of the same group,and PaCO2(45.04±12.12)mm Hg(1 mm Hg=0.133 kPa)of the experimental group was lower than that of the contorl group(61.54±11.93)mm Hg,and the difference was statistically significant(P<0.05).Before treatment,there was no statistically significant difference in forced expiratory volume in 1 second(FEV1),percentage of forced expiratory volume in FEV1(FEV1/FVC)and FEV1 as a percentage of the predicted value(FEV1%pred)between the two groups(P>0.05).After treatment,FEV1,FEV1/FVC and FEV1%pred of the two groups were higher than those before treatment of the same group,and the experimental group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Noninvasive ventilator ventilation shows affirmative efficacy for patients with with chronic obstructive pulmonary disease and respiratory failure,and it can effectively improve the blood gas indexes and lung function indexes.It is worthy of clinical promotion and application.
