近日节律对右美托咪定镇静和心血管效应的影响 被引量:1

Effects of recent circadian rhythms on sedative and cardiovascular effects of dexmedetomidine

摘要 目的探讨近日节律对右美托咪定镇静和心血管效应的影响。方法分析2016年7月至2017年2月于寿光市人民医院择期接受下腹部、下肢手术治疗的患者64例的临床资料,患者年龄18~65岁,其中33例于上午7∶00~11∶00接受麻醉者为A组,31例于下午13∶00~17∶00接受麻醉者为B组。分析两组患者各时点心率(HR)、脉搏血氧饱和度(SpO2)变化率和脑电双频指数(BIS)值。结果静脉泵注1μg/kg右美托咪定后,A组各时点HR、SpO2的变化率与B组比较差异未见统计学意义(P>0.05),A组T3~T6时点BIS数值分别为85.7±6.3、83.2±3.9、77.9±5.9、72.9±5.8,与B组比较差异有统计学意义(P<0.05)。结论近日节律可影响右美托咪定的镇静作用,但对右美托咪定的心血管效应未见明显影响。 Objective To investigate the effects of recent circadian rhythms on the sedative and cardiovascular effects of dexmedetomidine.Methods The clinical data of 64 patients who underwent surgical treatment of lower abdomen and lower extremity from Shouguang People’s Hospital from July 2016 to February 2017 were analyzed. The enrolled patients aged 18 to 65 years, 33 patients of which received anesthesia from 7∶00 to 11∶00 in the morning as group A, and 31 patients who received anesthesia from 13∶00 to 17∶00 in the afternoon were group B. The change rate of heart rate (HR) and pulse oxygen saturation (SpO2), bispectral index (BIS) value of the two groups were analyzed.Results After intravenous injection of 1 μg/kg dexmedetomidine, the change rates of HR and SpO2 in group A were not significantly different from those in group B at any time points (P>0.05). The BIS values of group A at T3 to T6 were 85.7±6.3, 83.2±3.9, 77.9±5.9, 72.9±5.8, respectively. There was a statistically significant difference in the BIS value between the two groups at the same point(P<0.05).ConclusionsThe recent circaclian rhythms can affect the sedative effects of dexmedetomidine, but has no significant effect on the cardiovascular effects of dexmedetomidine.
