甲亢患者血清促甲状腺激素水平与甲状腺激素及血糖、血脂代谢水平的相关性

Correlation between serum thyrotropin level and thyroid hormone,blood glucose and blood lipid metabolism in patients with hyperthyroidism

目的探讨甲亢患者血清促甲状腺激素(TSH)水平与甲状腺激素、血糖、血脂代谢水平的相关性。方法抽取2017年7月至2019年6月在驻马店市中心医院治疗的64例甲状腺功能亢进患者,根据患者的TSH水平,将其分为A组(n=30)与B组(n=34)。A组患者TSH≤0.02μIU/ml,B组患者TSH>0.02μIU/ml。所有患者均进行甲状腺功能指标、血糖、血脂代谢指标检测。结果A组患者血清总三碘甲状腺原氨酸(TT3)、总甲状腺素(TT4)、游离甲状腺素(FT4)均高于B组,但差异未见统计学意义(P均>0.05);游离三碘甲状腺原氨酸(FT3)水平高于B组(P<0.05);A组患者空腹血糖(FBG)高于B组,高密度脂蛋白胆固醇(HDL-C)水平低于B组(P<0.05);总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)指标比较,差异未见统计学意义(P>0.05)。结论甲状腺功能亢进患者血清TSH水平与甲状腺激素、血糖、血脂代谢水平均具有一定相关性,在进行甲状腺激素监测时,应同时进行血脂、血糖水平监测。

Objective To analyze the correlation between serum thyrotropin (TSH) level and thyroid hormone, blood glucose and blood lipid metabolism in patients with hyperthyroidism.Methods A total of 64 patients with hyperthyroidism treated in Zhumadian Central Hospital from July 2017 to June 2019 were divided into group A (n=30)and B group(n=34) according to their TSH level. The patients with TSH ≤ 0.02 μIU/ml were set as group A, and patients with TSH>0.02 μIU/ml as group B. The thyroid function, blood glucose and blood lipid metabolism of all the patients were examined.Results Serum total triiodothyronine (TT3), total thyroid hormone (TT4), free thyroid hormone (FT4) in group A were slightly higher than those in group B, but the difference were not statistically significant (P>0.05). The level of FT3 in group A was significantly higher than that in group B (P<0.05). Fasting blood glucose (FBG) level in group A was significantly higher than that in group B, but the level of high density lipoprotein cholesterol (HDL-C) was lower than that in group B (P<0.05);the total cholesterol (TC), triglyceride (TG) and low density lipoprotein cholesterol (LDL-C) was compared between the two groups the difference was not statistically significant (P>0.05).Conclusions There is a certain correlation between serum TSH level and thyroid hormone, blood glucose and blood lipid metabolism in patients with hyperthyroidism. Blood lipid and blood glucose should be monitored at the same time when thyroid hormone monitoring is carried out.
