缬沙坦联合氢氯噻嗪治疗肾性高血压对患者昼夜血压节律及血清omentin-1、氧化应激的影响 被引量:3

摘要 目的研究缬沙坦联合氢氯噻嗪治疗肾性高血压对患者昼夜血压节律及血清网膜素-1(omentin-1)、氧化应激反应的影响。方法随机抽取2017年1月至2019年1月河南大学淮河医院收治的肾性高血压患者200例,依据治疗方案将其分为两组,每组100例。研究组给予缬沙坦联合氢氯噻嗪治疗,对照组给予缬沙坦治疗。比较两组动态血压、血清omentin-1、氧化应激指标。结果治疗后,两组患者24 h收缩压、舒张压比较差异未见统计学意义(P>0.05),研究组白昼收缩压、舒张压高于对照组(P<0.05),研究组夜间收缩压、舒张压低于对照组(P<0.05)。治疗28 d后,研究组夜间血压下降>10%者占比高于对照组(收缩压80.00%比30.00%,舒张压76.00%比36.00%);治疗后,两组omentin-1、超氧化物歧化酶水平高于治疗前(P<0.05),丙二醛水平低于治疗前(P<0.05),且研究组变化幅度优于对照组(P<0.05)。结论缬沙坦联合氢氯噻嗪治疗肾性高血压可有效控制血压,改善omentin-1、氧化应激指标。 Objective To investigate the effect of valsartan combined with hydrochlorothiazide on circadian blood pressure rhythm, serum omentin-1 and oxidative stress in patients with renal hypertension.Methods A total of 200 patients with renal hypertension admitted to Huaihe Hospital of Henan University from January 2017 to January 2019 were randomly selected and divided into two groups according to the treatment plan, with 100 patients in each group. The study group was treated with valsartan and hydrochlorothiazide, and the control group was treated with valsartan. The ambulatory blood pressure, serum omentin-1, and oxidative stress were compared between the two groups.Results After treatment, there was no significant difference in 24 h systolic blood pressure or diastolic blood pressure between the two groups (P>0.05). Daytime systolic blood pressure and diastolic blood pressure were higher in the study group than that in the control group (P<0.05). The diastolic blood pressure was lower than that of the control group (P<0.05). After 28 days of treatment, the proportion of patients with a decrease in nighttime blood pressure> 10% in the study group was higher than that in the control group (systolic blood pressure: 80.00% vs. 30.00%, diastolic blood pressure: 76.00% vs. 36.00%), P<0.05. After treatment, omentin-1 and superoxide dismutase levels in the two groups were higher than that before treatment (P<0.05), malondialdehyde levels were lower than that before treatment in both groups (P<0.05);and the magnitude of change in the study group was better than that in the control group (P<0.05).Conclusions Application of valsartan combined with hydrochlorothiazide can effectively control blood pressure, improve omentin-1 and oxidative stress in patients with renal hypertension.
