自拟活血通利汤联合金黄跌打散外敷治疗气滞血瘀型急性膝关节创伤性滑膜炎临床观察

Clinical Effect of Self-drafting Huoxue Tongli Decoction Combined with Jinhuang Dieda Powder on Acute Traumatic Gonarthromeningitis with the Syndrome of Qi Stagnation and Blood Stasis

目的:观察自拟活血通利汤联合金黄跌打散外敷治疗气滞血瘀型急性膝关节创伤性滑膜炎的临床效果。方法:将气滞血瘀型急性膝关节创伤性滑膜炎患者64例,随机分为对照组和观察组各32例;对照组予关节腔穿刺抽取积液及口服艾瑞昔布片治疗,观察组在对照组基础上加用内服自拟活血通利汤及外敷金黄跌打散处理,对两组治疗后的疗效、膝关节功能KSS评分、患膝髌骨上缘周径以及膝关节疼痛数字评分(NRS)进行统计学分析。结果:两组治疗后总有效率差异无统计学意义(P>0.05);观察组的临床治愈率高于对照组(P<0.05);治疗后两组患膝周径及NRS低于治疗前,膝关节功能KSS评分高于治疗前,差异有统计学意义(P<0.05);且观察组患膝周径、NRS评分均低于对照组,KSS评分高于对照组,差异有统计学意义(P<0.05)。结论:自拟活血通利汤联合金黄跌打散外敷治疗气滞血瘀型急性膝关节创伤性滑膜炎效果确切,能够有效减轻膝关节软组织肿胀程度,缓解疼痛,改善膝关节功能,提高膝关节活动能力。

Objective:To observe the clinical effect of self-made Huoxue Tongli decoction combined with Jinhuang Dieda powder external application on acute knee joint traumatic synovitis of Qi stagnation and blood stasis type.Methods:64 patients with acute knee joint traumatic synovitis of Qi stagnation and blood stasis were randomly divided into a control group and an observation group with 32 cases each;the control group was treated with joint cavity puncture to extract effusion and oral irixibu tablets.In the observation group,the self-made Huoxue Tongli decoction and the external application of Jinhuang Dieda powder were added on the basis of the control group.The therapeutic effect,knee function KSS score,the circumference of the upper edge of the patella and knee pain in the two groups were treated after treatment Statistical analysis was performed using the numerical score(NRS).Results:There was no statistically significant difference in the total effective rate between the two groups after treatment(P>0.05);the clinical cure rate of the observation group was higher than that of the control group(P<0.05);the knee circumference and NRS of the two groups after treatment were lower than before treatment The KSS score of knee function was higher than before treatment,and the difference was statistically significant(P<0.05);and the circumference of the knee and NRS score of the observation group were lower than the control group,and the KSS score was higher than the control group,the difference was statistically significant(P<0.05).Conclusion:The self-made Huoxue Tongli decoction combined with Jinhuang Dieda powder external application is effective in the treatment of acute knee joint traumatic synovitis of Qi stagnation and blood stasis type,which can effectively reduce the degree of soft tissue swelling of knee joint,relieve pain,improve knee function and improve knee Joint mobility.
